POSITION DUTIES

This position provides support for the Howard County Health Department Community Engagement Team in the Bureau of Assessment, Planning and Community Engagement (APC), which includes the Howard County Local Health Improvement Coalition (LHIC). Responsibilities include program coordination, training and promotion of CAREAPP and related community-engagement initiatives, including but are not limited to strategic communication and engagement of HCHD staff, partners, and community residents.

This position is eligible for hybrid telework.

60% CAREAPP Engagement Coordination

  • Develops and recommends strategies for the maximum utilization of CAREAPP by HCHD and community partners, as well as other priority HCHD community engagement services;
  • Maintains liaison with CAREAPP vendor to ensure CAREAPP functionality and accessibility, community partner engagement, and impactful data metrics for evaluation and decision-making; 
  • Supports the development and implementation of ongoing CAREAPP trainings and technical assistance for HCHD program staff to use CAREAPP for client needs assessment, data metrics, resource identification and referral, and shared care coordination; to integrate into program workflows; and to support the onboarding process of new hires;
  • Provides targeted trainings for existing Howard County CAREAPP users and new users, including the Community Health Worker Network and internal and external partners. 
  • Reviews CAREAPP data and reports to track engagement progress, impact/outcomes, identify trends and to recommend strategies for improvement
  • Reviews and complies with program-specific (or bureau-specific) policies, procedures, program manuals and standing orders (if applicable) and provides feedback for quality improvement as requested. 

30% Community Outreach and Engagement Coordination 

  • Cultivates and maintains cooperative relationships with all HCHD Bureaus, CAREAPP vendor, and partner organizations, including LHIC participating organizations to ensure successful completion and collaboration for assigned programs; 
  • Communicates effectively with co-workers, management, and cross-Departmental partners; community partners, program participants, and general public;
  • Supports the coordination of specified HCHD community engagement programs, including strategic agenda; activity development; communication; planning and implementation of actions;
  • Supports the identification/evaluation of the effective use of educational materials and methods in community outreach and for promoting HCHD community engagement initiatives to meet the health education needs of the community or of a target population and ensure health awareness and self-directed health management for HCHD staff, visitors, and community members; 
  • Develops and provides accessible presentations/trainings and represents HCHD within community partner programs and outreach to the general public in coordination with Supervisor, LHIC, and other Bureaus.
10% Other
Performs other related duties as assigned, including but not limited to:

  • Accepts responsibility for special projects and other duties as assigned;
  • Participates in appropriate professional education and training for roles and responsibilities of work assignments, as requested or approved by Supervisor;
  • Communicates professionally with Supervisor, HCHD coworkers, MDH staff, and other partners and community agencies;
  • Complies with HCHD/MDH policies and procedures;
  • Participates in Emergency Preparedness trainings, exercises or actual emergencies;
  • Participates in seasonal flu clinics, as approved by Supervisor;
  • Serves on affiliated Health Department Committees as assigned.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

Desired qualifications
-Experience with online web based referral platform and data analytics.
-Experience conducting presentations to stakeholders, potential business partners, or the general public.
- Community outreach including capacity building.
- Experience collaborating virtually and in-person with a diverse team. 
- Knowledge about traditional and modern training methods and techniques.
