Main Purpose of Job

The main purpose of this position is to reduce relapse and recidivism among re-entering citizens living with co-occurring substance use and mental health disorders (CODs) and help them develop skills to build meaningful, purposeful lives in the community. The Re-entry Coordinator will use assertive outreach strategies to develop and strengthen systems linkages to enhance and expand access to a recovery-oriented continuum of care and coordinate with correctional facilities to transition participants to fully integrated, Re-Entry-oriented treatment and Recovery Support Services necessary to lay the groundwork for change, sustained recovery, and facilitate meaningful participation in the community. The Re-Entry Coordinator will help clients to make the transition from life in incarceration to life in the community, through the coordination of wrap-around recovery services.

 This position is Grant Funded. The Grant requires that candidates must possess a High School Diploma or GED.

MINIMUM QUALIFICATIONS

Education: Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: One year of administrative staff or professional work in a health care setting.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess the following:
- Knowledge or experience with re-entry services (providing coordinated care to justice involved individuals returning to the community)
- Experience working with individuals living with co-occurring substance use and mental health disorders
