Main Purpose of Job

This position is located within the Law Enforcement Assisted Diversion (LEAD) program within the Behavioral Health Division of St. Mary's County Health Department. The main purpose of this full-time position will be to plan and implement a wide variety of support activities for program participants and their families. The duties for this position are as follows:
  • Conduct outreach and maintain relationships with family members of individuals who are currently enrolled in the LEAD program in order to provide resources that support self-identified needs;
  • Provide linkages to care and other community resources/supports to assist the LEAD family members including but not limited to: housing, vocational and educational opportunities, treatment, mental and other health services, and community services;
  • Collect and maintain data on program participation, successes, challenges etc.; prepare reports based on relevant data;
  • Maintain confidential client information including casefiles;
  • Establish and maintain a comprehensive provider file for referrals to the appropriate resources.

MINIMUM QUALIFICATIONS

Education:  A Bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience:  Two years of professional experience in health services, one year must have been professional work related to treatment and services for mentally ill patients.

Notes:

1. Candidates may substitute the possession of a Master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2. Candidates may substitute the possession of a Bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services for the required education.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess:
  • Experience and/or working knowledge of St. Mary's County community resources and programs related to basic needs (housing, vocation, education etc.);
  • Experience working with a diverse group of people including clinical staff and/or law enforcement.
