Main Purpose of Job

The main role of this position as the Hub and Spoke Coordinator and Grant Monitor will provide grant monitoring, funding tracking, technical assistance and report completion for the Behavioral Health’s Hub and Spoke vendors, to facilitate the transfer of an OUD patient from a community-based prescriber (Spoke) to a SUD treatment provider (Hub) and to ensure the smooth transition back to the originating spoke if warranted. The grant monitor will assume primary grant monitoring duties, which include, documenting program progress and deliverables contained within the reporting tool that will be shared monthly and/or quarterly to the State Opioid Response (SOR) evaluation team, which include the Behavioral Health Administration (BHA)/Substance Abuse and Mental Health Service Administration (SAMHSA). In addition, the position will require grant, fiscal monitoring and data reporting for SUD residential referrals related to recovery residences and other relevant services.

POSITION DUTIES


20% Program Management 
  • Care Coordinator will create and maintain a directory resource directory, including special capacity participating Hub/Spoke providers in Howard County.
  • The employee will assume primary grant monitoring duties, funding tracking that include, grant monitoring for, Hub and Spoke, Recovery Residences, and SUD Residential Referrals.
  • Develops procedures for improved services, coordination and expansion of existing services and development of new services
  • Care Coordinator will oversee, data collection, technical assistance and quality assurance efforts related to Hub and Spoke and other projects.
  • Performs other related duties.
  • Submit project updates and programmatic data monthly to the Bureau Director and the Behavioral Health Administration as required by due dates.
  • Attend internal/external meetings and trainings relevant to program activities  
  • Responsible for planning and organization of special projects and other duties as assigned. 

70% Grant Monitoring of Contracts/Tracking of Funding
  • Grant monitoring of contracts and funding tracking for Hub and Spoke and other responsibilities which include Recovery Residences, and SUD Residential Referrals.
  • Submit program relevant data monthly/quarterly/bi-annually to the Behavioral Health Administration (BHA)/Bureau Director by due dates.
  • Submit monthly or as requested Hub and Spoke to the University of Maryland and/or Behavioral Health Administration.
  • Submit monthly or as requested Hub and Spoke, and other projects/grants relevant to the mission of the Bureau.
  • Address contract vendor questions is the point of contact for the vendors, and provide technical assistance. 
  • Monitor contractors to ensure in compliance with conditions of award, performance standards, expense limits, and service provisions. Conduct grant monitoring/site visits quarterly/bi-annually or as the monitoring schedule indicates. Submit audit documentation to the Behavioral Health Administration (BHA)/Bureau Director by due dates. 
  • Track total expenses utilized by contracted services in scope of work.  Coordinate with Fiscal Services regarding referrals and invoicing as needed.
  • Review/approve invoices associated with contracts in preparation for payment. Works with Fiscal Services/Contracts Unit to address any concerns with invoices, contracts, or purchase orders. Submit invoices to the Bureau Director for payment approval after ensuring the contract and purchase order have been executed and funding is sufficient.   
  • Track contract expiration and renewal dates to mitigate gaps in services.  Work with the Contracts Unit  to ensure contract creations, renewals and purchase orders are completed by due dates. Informs the Bureau Director of any contract creation/renewal/purchase order concerns, as soon as possible.
  • Participate in Emergency Preparedness drills or actual events.  
  • Draft new contract worksheets with new vendors. Work with the Contracts Unit to ensure contracts are drafted within specification.  Gain final contract approval from Bureau Director.
  • Lead efforts working with vendors/Fiscal services to ensure 432 (budget) packets and 440s (performance measures are submitted on time and adequately completed.

10% Other Duties Assigned
Performs other related duties, including but not limited to:
  • Accept responsibility for special projects as assigned. 
  • Participate in appropriate professional education and training for roles and responsibilities of work assignments, as requested or approved by Supervisor.
  • Participate in Emergency Preparedness drills or actual events.  

MINIMUM QUALIFICATIONS

Education:   A Bachelor’s degree from an accredited college or university in nursing, social work, psychology, education, counseling or a related field.

Experience: Three years of professional experience in health services, two years must have been in the Health Services option.

Notes:

1. Candidates may substitute the possession of a Master’s degree from an accredited college or university in a health or      human service field for one year of the required general experience.

2. Experience in the Health Services option must be as follows: professional clinical or administrative work in health or medical services in areas other than Mental Health, Developmental Disabilities or Addictions.

3. Candidates may substitute the possession of a Bachelor’s degree from an accredited college or university in another field plus one additional year of professional experience in health services for the required education.

4. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care   Administration classifications or Civil Affairs Specialist specialty codes in the health related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

Preferred candidates should possess the following:

  • Experience with grant monitoring, which includes, but is not limited to tracking funds, data reporting, auditing, planning and implementation 
  • Experience working with individuals with mental health and substance use
  • Experience collaborating with community stakeholders in a public health setting 
  • Experience using electronic health record (EHR) systems and managing consumer cases/services
  • Excellent organization, written and oral communication skills 
  • Ability to prioritize work and complete tasks efficiently to meet deadlines
