    ABOUT OUR TEAMS:
The Assertive Community Treatment (ACT) team is a multidisciplinary clinical team that delivers comprehensive services to clients who are diagnosed with severe and persistent mental illness and whose needs have not been met by more traditional service delivery approaches. Individuals served by the ACT team have often experienced frequent psychiatric hospitalization and use of hospital emergency rooms, involvement with the criminal justice system, substance abuse, and lack of engagement with traditional outpatient services. By basing treatment in the community, the team can better assist the client with challenges they face. The team provides community support services interwoven with treatment and rehabilitative services and regularly scheduled team meetings with the goal of individualizing client care.

The Community Support Program (CSP) is targeted to individuals who need the most intense level of community-based care. The program utilizes a team approach to provide intensive, rehabilitative community support, crisis intervention, individual and group skill-building, also known as recovery education.  The majority of the interventions are community-based, delivered in the individual’s home, neighborhood or community, which enables the team to become intimately familiar with the individual’s surroundings, strengths and challenges, within the context of their environment.  The desired outcome is to assist individuals toward an independent, enriched life based on their own choices and preferences.

    MORE ABOUT THE DIVISION:
SWCMHS is responsible for the oversight and management of contracts with private not-for-profit agencies and residential providers providing mental health services in the southwest region of the state.  With locations in Bridgeport and Stamford, SWCMHS provides inpatient, outpatient and assertive community treatment (ACT) services and community support services for adults with severe behavioral health disabilities; mental health services for people who are deaf or hearing impaired and a mobile crisis intervention team. SWCMHS also provides outreach to people who are homeless and mentally ill. Special agency programs include Shelter Plus Care, a housing program for people who are homeless and who have a history of substance abuse or mental illness or who have AIDS.
