The State of Connecticut, Department of Developmental Services (DDS) – West Region, Private Administration – is accepting applications for two (2) Clinical Nurse Coordinator positions located in Cheshire and Waterbury, CT.
We are seeking a compassionate and highly organized Clinical Nurse Coordinator to lead and support the delivery of comprehensive, person-centered healthcare services. In this role, you will coordinate clinical programs, conduct health assessments, develop and monitor individualized healthcare plans, and collaborate with interdisciplinary teams to ensure continuity of care across treatment and community settings.
The Clinical Nurse Coordinator serves as a vital resource for staff, families, and community partners while advocating for individuals to receive services in the least restrictive and most supportive environments possible. This position also includes program development, clinical oversight, community outreach, documentation, reporting, and ongoing consultation to promote quality outcomes and reduce recidivism. The ideal candidate is a skilled nursing professional with strong clinical judgment, communication, and leadership abilities who is committed to improving the health and well-being of the individuals served.
