THE POSITION
HIGHLIGHTS

- UNIT: Intensive Clinical Team
- SCHEDULE: Full-Time, 40 Hours per Week, First Shift, 8:00am - 4:30pm, Monday through Friday

Responsibilities include, but are not limited to, the following:
- Complete treatment plans and providing case management;
- Intake, assessment, and ongoing evaluations of clients;
- Work with clients and team members in the development of Individualized Recovery plans and supporting documentation;
- Facilitate psychoeducation, therapy or skill building groups as well as providing limited individual and family therapy;
- Provide culturally sensitive and evidence-based treatments such as DBT, EMDR, IDDT and Trauma treatment and works in the community for most services;
- Conduct community visits to assess individuals in psychiatric crisis situations;
- Represent the program at State and local hospitals, probate courts, and community settings;
- Assist team members in the development of recovery plan and may provide clinical consultation to team cases and supervision of other team members and students;
- Participate in and coordinate all treaters' meetings, case conferences critical incident reviews and other meetings as indicated;
- Work as a member of a multidisciplinary team to perform risk assessments and functional assessments;
- Make appropriate and timely referrals and build relationships with service providers and natural supports; and
- Document all services in the medical record in a timely manner.

DMHAS is a health care agency whose mission is to promote the overall health and wellness of persons with behavioral health needs through an integrated network of holistic, comprehensive, effective, and efficient services and supports that foster dignity, respect, and self-sufficiency in those we serve.
WCMHN was formed in 1996 in order to coordinate clinical, fiscal and administrative oversight of Region 5 contracted and state-operated mental health services. Their mission is to provide a comprehensive and effective recovery-oriented system of mental health and addiction services that promotes self-sufficiency, dignity, cultural sensitivity and respect.
Within this service system, there are three state-operated Local Mental Health Authorities, which are located in Waterbury, Danbury and Torrington. In partnership with WCMHN, they provide and coordinate local clinical services. Over 200 staff including psychiatrists, nurses, social workers, rehabilitation and mental health workers provide services for persons with a long-term mental illness. Approximately 7,000 clients receive services in a variety of programs, including peer support programs, recovery services, supported employment, homeless services, residential, crisis services, jail diversion, outpatient, case management, Assertive Community Treatment Teams (ACTT) and young adult services.
The Intensive Clinical Team's best practice model of intensive community care with an emphasis on assisting clients to gain or improve skills for successful living in the community, an improving quality of life, and greater independence to pursue their goals through values of recovery. As part of the intensive clinical team, it provides clinical, case management rehabilitative and recovery-oriented services to an assigned caseload of individuals with serious mental health and/or co-occurring substance and medical disorders.

PURPOSE OF JOB CLASS (NATURE OF WORK)
In the Department of Mental Health and Addiction Services community based behavioral health program, this class is accountable for providing clinical assessment and evaluation of treatment programs for an on-going primary caseload of clients for a behavioral health treatment team.
EXAMPLES OF DUTIES
- Functions primarily to implement treatment plans for clients on caseload;
- Provides case management services to clients;
- Assists clients in linking with community support systems and agencies;
- Monitors clients progress within mental health and community support systems;
- Performs intake, evaluation and assessment of treatment programs under supervision of a licensed independent practitioner;
- Participates in counseling sessions;
- Provides education and training to clients in community living and activities of daily living skills;
- Monitors clients medication compliance and coordinates meetings with team psychiatrists or nurses as necessary;
- Provides appropriate interventions to clients in crisis including outreach, counseling and/or referral to other service providers;
- Completes all required documentation related to client treatment in accordance with established procedures;
- May transport clients to and from appointments;
- May assist in development of treatment plans for new clients;
- May administer medication;
- May provide nursing consultation and/or input; performs related duties as required.
KNOWLEDGE, SKILL AND ABILITY
- Knowledge of
- behavioral health counseling techniques;
- social work methodology, casework, group work and community mobilization;
- family dynamics and dynamics of interpersonal relationships;
- social, cultural, economic, medical, psychological and legal issues which influence attitudes and behaviors of clients and families;

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE
A Bachelor’s degree in a health care field and two (2) years of experience working independently using clinical case management techniques in a behavioral health setting.
NOTE: Case management is defined as primary responsibility for the implementation, monitoring, evaluation and assessment of client progress towards meeting treatment objectives as outlined in an approved treatment plan.
MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED
- A Master’s degree in counseling, psychology or other clinical discipline may be substituted for one (1) year of the General Experience.
- A current Connecticut license as a Registered Nurse OR hold a Multistate or Compact Registered Nurse license and two (2) years of experience working in a psychiatric setting may be substituted for the Bachelor’s degree.
- A Master’s degree in social work (MSW) and two (2) years at the level of a Mental Health Assistant 2 independently performing the full range of paraprofessional case management in a behavioral health setting may be substituted for the General Experience.
PREFERRED QUALIFICATIONS
- Experience providing clinical outpatient treatment in the community for adults with severe and prolonged mental illness, addiction, and co-morbid medical conditions required
- Experience completing risk assessments and developing risk mitigation plans required
- Experience performing intake (psychosocial) assessments and person-centered recovery planning
- Experience providing evidence-based individual, family and group treatment
SPECIAL REQUIREMENTS
- Incumbents in this class must possess and retain licensure or certification in designated positions.
- Incumbents in this class may be required to be a certified substance abuse counselor.
- Incumbents in this class may be required to have knowledge of sign language and communication ability with the deaf and hearing impaired in designated positions.
- Incumbents in this class may be required to have ability in Spanish oral and written communication in designated positions.
- Incumbents in this class may be required to travel.
