The State of Connecticut, Department of Public Health (DPH) is hiring! We are seeking a qualified individual for the position of Health Program Associate (Diabetes Coordinator). This position will work within the Diabetes Prevention and Management/Heart Disease Stroke Prevention Program of DPH.
This Health Program Associate will be responsible for coordinating, strategic planning, monitoring, and supporting the evaluation of statewide diabetes prevention and self-management strategies including those supported by a federal grant. The Diabetes Coordinator will use clinical expertise and experience to provide technical assistance to implementing contractors and partners to assure activities reflect current best practice and support Centers for Disease Control and Prevention-identified strategies and outcome goals. This position will serve as the diabetes subject matter expert for the Community, Family Health and Prevention Section.

The work schedule for this position is typically Monday - Friday, 35 hours per week. This position may be eligible to telework up to 50%.

One (1) year of the General Experience must have been in health care program administration, a health systems agency or other health care organization in at least one of the following areas:
1. grant writing or monitoring;
2. formal program planning, development, management or evaluation;
3. or program consultation.
For State Employees this is interpreted at the level of Health Program Assistant 2.
Note: A health care organization is defined as a large multi-dimensional agency with responsibility for developing health programs.
