Under general supervision, to provide program planning, development, review and evaluation of clinical and administrative activities in assigned program(s); direct, coordinate and oversee the nursing and other health care services of a program to clients across the age spectrum and/or targeted populations; to supervise a multidisciplinary team including professional, para-professional support staff, nursing student and interns; to ensure program regulations and procedures are followed; and to do related work as required.  




These positions are located in the Health Care Services Agency and Community Development Agency and report to either a Director or Deputy Director of Public Health Nursing, Physician V, Physician IV, Registered Nurse V, Division Director, Health Care Program Administrator II or Program Director.  The Registered Nurse IV exercises functional and administrative supervision over a program(s) including the professional, para-professional, support staff, nursing students and interns.  The Registered Nurse IV is distinguished from the lower-level class of a Registered Nurse III in that the latter acts in a lead capacity, while the former has primary supervisory responsibilities.  The Registered Nurse IV is further distinguished from the next higher-level class of Registered Nurse V in that the latter is responsible for managing the nursing services of a major functional area and/or program (s) and/or assists in the overall management of a Division.


NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.


1. Promotes identification of service interventions needed to assign appropriate staff by using established workload standards and/or acuity levels for clients and communities.

2. Collaborates with other departments and agencies for the provision of services to clients and families.

3. Develops, implements and manages service delivery processes; interprets and implements policies, procedures, and regulations; and ensures regulatory compliance.

4. Participates in Public Health emergency response activities via department-wide or program-specific on-call rotation.

5. Assures the establishment and maintenance of the organizational structure of a program, which maximizes utilization of personnel and supplies.

6. Participates in the budget development process, monitors and authorizes expenditures; contract procurement; ensures the maintenance of equipment and supplies; oversees the claim reimbursement process.

7. Participates in the establishment of program and division goals and objectives and promotes their achievement; develops, implements and monitors program performance measures.

8. Participates in the determination and implementation of client care, documentation standards, quality assurance program criteria, and monitors the documentation of their achievement and compliance.

9. Provides clinical assistance/oversight of staff performing case management and care coordination.

10. Assumes leadership role in monitoring the provider network.

11. May render public health nursing services, tests and assessments.

12. Supervises, recruits, trains, evaluates and disciplines registered nurses/public health nurses, professional, para-professional, support staff, nursing students and interns.

13. Prepares documentation to support requests for continuing enhancement of program capabilities and makes recommendations for personnel supplies, equipment and facility needs.

14. Conducts and/or participates in meetings with designated staff, community meetings, interagency case conferences, forums and/or focus groups.

15. Identifies data information needs and works closely with information systems to facilitate the collection of data.

16. Monitors programs for standards of care, education to clients and communities and coordination of other resources.

17. Contributes to the planning and implementation of continuing educational experiences for employees and students.

18. Investigates, gathers data and responds to issues and grievances; escalates matters as appropriate.

19. May monitor the operation of the infection control activities within program for clients, employees and the community as appropriate.

20. Directs and provides patient education and informational materials to drive behavior change and improve health.

21. Conducts and/or participates in research and studies related to the assigned program.

22. Prepares a variety of correspondence, narrative and statistical reports, program documentation, policies, procedures and other written materials.

23. Acts as a liaison between staff, nursing administrator, and/or Division Director.




Possession of a bachelor’s degree in Nursing from a recognized four-year college or university (180 quarter-units or semester 120 units).



The equivalent of three years of full-time, recent experience within the last five years as a Registered Nurse III in Alameda County classified service.



The equivalent of four years of full-time, recent experience within the last five years as a Registered Nurse II in the Alameda County classified service.



The equivalent of five years of full-time, recent nursing experience within the last five years in a clinical and/or public health setting, which has included at least one year of full-time lead/supervisory experience.



Possession of a master’s degree in business administration, public administration, health/healthcare management/administration, nursing or public health from an accredited college or university may be substituted for two years of experience in any pattern above.



  • Possession of a current and valid license to practice as a Registered Nurse in the State of California. 
  • Possession of a valid Basic Life Support for Healthcare Providers certificate, which includes cardiopulmonary resuscitation (CPR) and Automated External Defibrillator (AED) training.
  • Some positions may require possession of a current and valid Public Health Nursing (PHN) Certificate issued by the State of California Board of Registered Nursing.
  • Some positions may require a valid California Motor Vehicle Operator’s license.


Special Requirements:

  1. In compliance with the Administrative Simplification provision of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), employees in this classification are required to possess a National Provider Identifier (NPI) number prior to their first day on the job.
  2. In Compliance with Medicare regulations, employees in this classification are required to complete the “Medicare Enrollment Process for Physicians and non-Physician Practitioners” through the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services, prior to their first day on the job. Active enrollment in Medicare is a condition of employment. Failure to attain or maintain active enrollment will result in termination.
