Under general supervision, to provide a variety of nursing care services, case management, treatment plan, teaching, counseling and referral to clients and families in a home and/or community setting, to provide nursing assessment, planning, intervention and evaluation; coordinates care with other health care providers, professionals and other agencies and community representatives; to assist in the prevention of communicable diseases; and to perform related duties as required.


Registered Nurse I/II positions are located in the Health Care Services Agency, Social Services Agency and Community Development Agency.

Registered Nurse I: This is the entry-level class in the Registered Nurse series. Under close supervision, a Registered Nurse I performs professional nursing duties. This class is the training level and incumbents are expected to gain experience and demonstrate proficiency which qualifies them to promote to the higher level after six months of full-time experience. Appointment at this level will not be extended beyond six months.

NOTE: The following are the duties performed by employees in this classification. However, employees may perform other related duties at an equivalent level. Each individual in the classification does not necessarily perform all duties listed.

1. Conducts home visits to clients and families for a variety of referrals; and gathers and assesses information about clients using the nursing process to meet the nursing needs of clients.
2. Provides nursing case management and assessment; monitors clients for significant and/or critical changes; and documents all interventions provided to clients, families and groups.
3. Educates clients and families about health care and disease prevention techniques to promote health and well-being; interprets clinical data procedures, regimens and services to clients and families; and prepares educational materials and other resources.
4. Coordinates and communicates with health care providers, professional and other agencies and community representatives.
5. Conducts contact investigation to determine high risk exposure; refers contacts for screening and treatment and follows up to assure appropriate treatment was received.
6. Services as a client advocate; assists clients with accessing health care providers and/or services; accompanies clients to appointments with other providers; and follows up on appropriate referrals to address unmet health care needs.
7. Reviews and triages confidential morbidity and laboratory reports; reviews epidemiological investigations; and conducts surveillance activities for communicable diseases and vaccine preventable diseases.
8. Provides direct immunization services to children, adolescents and adults.
9. Initiates required nursing procedures to ensure patient health and safety.
10. Develops, modifies and implements a nursing care plan, appropriate to the needs of the client, to ensure timely and appropriate assessment, diagnosis, treatment and evaluation.
11. Participates in the implementation of patient care standards, infection control standards and quality assurance criteria to ensure appropriate level of clients' care.
12. Participates in community outreach and health education activities for patients and families and promotes utilization of preventive health services and positive health behaviors.
13. Interprets policies, procedures and regulations to meet standards set by State and National regulatory and accreditation agencies.
14. Provides guidance to ancillary workers for a given period of time by organizing, adjusting and accepting responsibility of activities to ensure quality of care is provided to clients.
15. Participates in staff meetings, health fairs, community meetings, interagency case conferences, forums and/or focus groups.
16. Conducts visual assessments of the environmental conditions in the home.
17. Conducts a baseline assessment of the client's psycho-social needs.

For Both Levels I & II


- Possession of a current and valid license to practice as a Registered Nurse in the State of California.
- Some positions may require possession of a current and valid Public Health Nurse (PHN) Certificate issued by the State of California
- Some positions may require a valid California Motor Vehicle Operator's license.

Special Requirements:

1. In compliance with the Administrative Simplification provision of the Health Insurance Portability and Accountability Act of 1996 (HIPAA), employees in this classification are required to possess a National Provider Identifier (NPI) number prior to their first day on the job.

2. In Compliance with Medicare regulations, employees in this classification are required to complete the "Medicare Enrollment Process for Physicians and non-Physician Practitioners" through the Centers for Medicare and Medicaid Services (CMS) of the U.S. Department of Health and Human Services, prior to their first day on the job. Active enrollment in Medicare is a condition of employment. Failure to attain or maintain active enrollment will result in termination.
