GENERAL STATEMENT OF DUTIES
The Outreach Worker will report to the Prevention Section Chief. They will work with the Prevention Section Team in conducting outreach in areas of Waterbury that have been identified as high-risk areas through data collection. They will assist in the education of Naloxone administration and Substance Use Disorder, for community partners and community members throughout the city of Waterbury. Outreach Workers will assist individuals with substance use disorders with connections to care, using available and harm reductions supplies such as Naloxone and Fentanyl testing strips.
EXAMPLES OF WORK
ESSENTIAL FUNCTIONS:
•Conduct outreach with team members in identified areas, to educate and inform different populations on Naloxone and substance use disorder.
•Conduct trainings and presentations for substance use disorder and Naloxone administration for community members and partners.
•Attend community events with Waterbury Health Department, City of Waterbury and community partners to speak with attendees regarding substance use disorder and Naloxone and educate them on program.
•Maintain repour with individuals’ diverse communities while ensuring dignity, cultural competence and respect is given to all community members.
•Assist individuals with harm reduction supplies.
•Assist individuals with connection to care with available resources through the Waterbury Health Department.
•Maintain good relationship and collaboration with community partners through events, trainings and presentations.
•Attend community partner meetings regarding substance use/mental health disorders.
•Maintain clear and detailed data collection information regarding trainings, attended events, presentations, harm reduction supplies handed out and connections to care identified using Microsoft programs and other programs used.
•Collaborate with team to create PSAs, Flyers, Posters and Campaigns for substance use and stigma education.
•Other duties deemed necessary by the Prevention Section Chief
REQUIRED KNOWLEDGES SKILLS AND ABILITIES
REQUIRED EXPERIENCE AND TRAINING
IN ORDER TO BE CONSIDERED FOR THIS POSITION YOU MUST INDICATE ON YOUR APPLICATION THAT YOU HAVE THE FOLLOWING EXPERIENCE (MINIMIM QUALIFICATIONS): High School Diploma, Minimum of 1-year experience working in high risk areas with high risk populations; Must have exceptional computer knowledge of Microsoft programs such as 365, Word, Excel, PowerPoint, Teams, etc.
Community Health Worker- Preferred but not essential.
The ability to communicate clearly and effectively. Experience building repour with diverse populations, while responding to cultural and social issues while maintaining respect, dignity and cultural competence. The ability to identify and acknowledging own biases.
EDUCATIONAL DOCUMENTS: Please submit a copy of Degree or Transcripts at time of application
Must be in possession of a valid Motor Vehicle License with a good driving history. (See statement below) In order to verify possession of the appropriate Driver’s License and good driving history, applicants will now be required to provide the H.R. Office with a Driving History Report that is less than 30 days old. Only reports generated by the Department of Motor Vehicle will be accepted.
ADDITIONAL REQUIREMENT
CONTINUING EDUCATION REQUIREMENTS: Attend trainings provided by Connecticut Department of Public Health and Center for Disease Control regarding, but not limited to, substance use disorder prevention, intervention and harm reduction. Attend trainings offered in Prevention and to the Prevention Section for performance enhancement and professional development by other entities and community partners.
ADDENDUM
This position is funded by a grant that is conditioned upon continuation of those funds. Additionally, the funding sources, in whole or in part, are other than City appropriations and may terminate when funding is no longer available.
