PRIMARY FUNCTIONS
This is a responsible position in the Health Department. Work includes identifying, follow up and home visits to individuals in New Haven who are inadequately immunized.
The role supports the Connecticut Immunization Action Plan, which engages in activities to improve vaccine delivery, tracking and referrals, education and assessment, training and support in the New Haven area. The goal is to achieve the National Healthy People 2030 objective of having all Connecticut children age-appropriately vaccinated by twenty-four months.
This position is 19 hrs/week, typically within Mon-Fri, 9am-5pm, but some evenings and weekends may be required to support programming.
TYPICAL DUTIES AND RESPONSIBILITIES
- Identifies through outreach, individuals living in New Haven who are inadequately immunized or lack adequate access to health care.
- Assists local health care facilities with immunization tracking systems, assessments and data entry.
- Conducts immunization assessments at local health care sites.
- Promotes access to the Health Department’s services through community outreach, organizing and group facilitation, distribution of education materials and resources on immunization and disease prevention and community presentations and health fairs.
- Performs other related work as required.
EDUCATION, QUALIFICATIONS & EXPERIENCE
Graduation from an accredited college, preferably with a BS degree in Public Health; or pursuing a BS degree in Health and Human Services; three years’ experience working with health care providers individuals and children; or any equivalent combination of training and experience which provides the following knowledge, skills and abilities:
KNOWLEDGE, EXPERIENCE, SKILLS & ABILITIES
- Comprehensive knowledge of health care community resources.
- Knowledge of adult and pediatric health care.
- Maturity and experience in successfully working with infants, young children and adults.
- Knowledge of computers, specifically Microsoft Windows.
- Ability to work with diverse socio-economic and socio-cultural groups.
- Ability to work well with co-workers and the public.
- Ability to communicate effectively both orally and in writing.
- Spanish / English bilingual preferred.
NECESSARY SPECIAL REQUIREMENTS
- Must have valid Connecticut Driver’s License and own transportation.
SALARY, BARGAINING UNIT & TERMS OF EMPLOYMENT
This is a Special Fund, "Non Tested" position. Continued employment contingent upon availability of future funding. Currently, funding is expected through September 30, 2028, however may be terminated at any time.
This part time position is not eligible to receive medical benefits, vacation pay, union membership, or union contractual benefits. Any period of part time employment will not be construed in any way to mean probationary employment. Persons may not be employed in two concurrent part time positions with the City and/or the Board of Education at the same time.
Employment is contingent upon the successful completion of: (1) a pre-employment background check, and (2) a pre-employment physical examination, including pre-employment drug screening accordance with the City’s Drug Free Workplace Policy*.
