Introduction

The New Haven Department's Immunization Action Plan program (IAP) works with physician offices to ensure that all area children are being immunized according to schedule. The IAP coordinator and outreach staff work to perform the following objectives:

  • Conduct outreach and track results using the statewide immunization information system (IIS) known as “CT WiZ”.  
  • Educate the public about the importance of vaccinations.
  • Conduct assessments in pediatric practices to improve vaccine delivery and storage.
  • Raise the immunization rates in New Haven.
  • Link children without a medical home to a medical home so their immunizations may be up-to-date.

PRIMARY FUNCTIONS

This is a responsible position in the Health Department. Work includes identifying, follow up and home visits to families in New Haven with children who are inadequately immunized.

The position will be responsible for promoting access to the Health Department’s services through community outreach, as well as parent organizing and group facilitation, distribution of education materials and resources on childhood immunization and disease prevention and community presentations and health fairs. Individuals will work under the direct supervision of the Pediatric Immunization Coordinator or designee.

TYPICAL DUTIES AND RESPONSIBILITIES

  • Assists local health care facilities with immunization tracking systems, assessments and data entry.
  • Conducts immunization assessments at local health care sites.
  • Other responsibilities will be to identify through outreach, families living in New Haven with children who are inadequately immunized or lack adequate access to health care because they do not have medical insurance.

EDUCATION, QUALIFICATIONS & EXPERIENCE

Graduation from an accredited college, preferably with a BS degree in Public Health; or pursuing a BS degree in Health and Human Services; three years’ experience working with health care providers and families with young children; or any equivalent combination of training and experience which provides the following knowledge, skills, and abilities:

  • Comprehensive knowledge of health care community resources.
  • Knowledge of pediatric health care.
  • Maturity and experience in successfully working with infants and young children.
  • Knowledge of computers, specifically Microsoft Windows.
  • Ability to work with diverse socio-economic and socio-cultural groups.
  • Ability to work well with co-workers and the public.
  • Ability to communicate effectively both orally and in writing.
  • Spanish / English bilingual skills highly desired.

NECESSARY SPECIAL REQUIREMENTS

  • Must have valid Connecticut Driver’s License and own transportation.

SALARY, BARGAINING UNIT & TERMS OF EMPLOYMENT

This is a Special Fund, "Non-Tested" position. Continued employment contingent upon availability of future funding.

This position is included in a collective bargaining agreement with Local 884, American Federation of State, County and Municipal Employees; the salary is within Local 884, Range 8.

Special Funded employees shall not be covered by the City pension but shall continue to be covered by Social Security.

Employment is contingent upon the successful completion of: (1) a pre-employment background check, (2) a pre-employment physical examination, including pre-employment drug screening in accordance with the City’s Drug Free Workplace Policy*, and (3) a 90-day probationary period.
