The Tucson Fire Department is seeking a motivated, dependable, and experienced professional to serve as a Community Services Outreach Specialist for the departments TC3 Program. This position is a civilian para-professional with social service/behavioral health expertise that involves effectively interacting in a "one-on-one capacity" with community members and agencies. This work involves varying hours and days.

General Description

This position will be a working with the department’s Tucson Collaboration Community Care program (TC3) and the Community Services Outreach Coordinator. The intent of the program is to provide call mitigation of unnecessary 911 calls and redirect these callers to the appropriate social service agency within the community. Reducing unnecessary 911 calls is part of the department’s resource and service efficiency plan. This position will process referrals from field crews and manage a case load of clients consisting of aging, medical, behavioral health and substance abuse with various and dynamic needs.

A Community Services Outreach Specialist is a civilian para-professional with social service/behavioral health expertise. The position involves effectively interacting in a “one-on-one capacity” with communities members and agencies. The work involves varying hours and days as required by the many different TC3 calls.

Reports to Fire Captain

Essential Functions

Process referrals by inputting client referrals as received from field crews each day. Research client information on the Health Information Exchange, AHCCCS portal and Electronic Patient Care Reports and any other means at your disposal

Process client intakes by meeting with clients on the phone or in their home to gather basic information including but not limited to perceived needs, home income, health insurance information, primary care information and case worker information. Includes home safety check and attempt to meet with any other important parties
 
Resource outreach by creating and maintaining relationships with carious agencies within the community that can provide TC-3 clients with alternatives to the 911 system to more appropriately manage each individual's particular circumstances

Provide case management through Identifying and coordinating with community resources in the area that will likely meet your client's needs. These should be alternatives to the 911 system and can include things like ongoing case management, durable medical equipment or medical and behavioral avocation

Provides case documentation by documenting client information, ongoing case notes and any other interactions and actions taken on the client's behalf including but not limited to speaking with primary care doctors, completing referrals to outside agencies and information obtained from outside agencies

Performs other related duties as assigned or requested. Any one position may not include all of the duties listed. This job classification specification identifies the responsibilities and tasks typically associated with the performance of the position.

Minimum Qualifications

EDUCATION LEVEL & TYPE: 6 months post High School

EXPERIENCE: At least 2 years experience in social services, behavioral health, counseling, Emergency Medical Services or closely related field

Valid and Unrestricted Class D - Driver's License

Special Job Requirements

LICENSURE/CERTIFICATION: Arizona licensed Emergency Medical Technician

Applicants who meet the minimum qualifications will be evaluated on the education and work experience in their employment profile and the responses to their supplemental questions. The highest scoring applicants will then be invited to participate in an Oral Board interview.

Oral Board Interviews will be held March 22, 2023

An applicant's ranking on the Civil Service list will be based on their final score which will be calculated as 20% of your rating for Education and Experience as outlined in the application supplemental questions, and 80% of your rating for the Oral Board Interview.

Veterans, Native American, or Disability preference points will be added to the final score for those that are placed on the Civil Service Employment List. If you qualify for preference points as outlined in the Employment Profile, you may email or upload your documentation. The DD 214's must be the member 4 copy which indicates characterization of service.
