The City of Tucson is seeking a motivated and passionate Care Coordinator to join the newly established Community Safety, Health and Wellness Office. General Description

The Community Safety, Health & Wellness Care Coordinators provide follow-up to non-emergency 911 calls and referrals from City of Tucson departments to support residents in accessing the appropriate social service agency within the community, reducing barriers to their ability to seek out necessary supports and preventing future crises. Care Coordinators act as a social worker/case manager to facilitate the individual or household getting and remaining connected to reliable City and community resources.

This class is supervised by the Community Safety, Health, and Wellness Resource Manager.

Essential Functions

This class will process referrals from field crews and manage a caseload of clients that may include various and dynamic needs related to aging, homelessness, behavioral health and substance misuse, among others.

The Community Safety, Health and Wellness Care Coordinators shore up gaps as needed and interface with existing field teams that fall under the Community Safety, Health and Wellness umbrella, including: 
  • Mental Health Support Team (MHST) 
  • Substance Use Response Team (SURT) 
  • Housing First Program (Homeless Services & Homeless Protocol) 
  • Tucson Collaborative Community Care (TC-3) 
  • Homeless Outreach Team  
Responds to assistance requests for service from local public safety organizations and the public.  Provides case management through identifying and coordinating community resources to meet client needs, including appropriate social service and/or community-based supports.   

Processes client intakes by meeting with clients to gather information including but not limited to perceived needs, income, existing case worker information and other social, mental, physical or economic current supportive services.  

Oversees case data collection and maintains accurate records.  

Works with management and partner teams in coordination of and service to the Community Safety, Health & Wellness program.  

Participates in neighborhood meetings, public meetings, other public forums and team meetings as needed.  

Networks with other City of Tucson departments and local nonprofit agencies and coalitions as needed.  

Attends trainings and participates in continuous quality improvement of the program.  

Performs other related duties as assigned or requested. Any one position may not include all the duties listed. This job classification specification identifies the responsibilities and tasks typically associated with the performance of the position. 

Minimum Qualifications

Minimum Education Level & Type: Bachelor’s Degree in Social Services, Public Health, Human Relations or closely related field or equivalent

Minimum Qualification Experience: 2 years of work or volunteer experience in direct service provision, case management or equivalent personal experience navigating services.    

Driving Requirement (Level/Type/CDL Endorsements): Valid and Unrestricted Class D - Driver's License.  

Preferred Qualifications

- Fluency in Spanish, Arabic or French

- Ability to: 
React quickly and calmly in emergency situations  
Center the needs of the client and act with compassion while maintaining personal boundaries   

- Knowledge of: 
Applicable safety requirements 
Mainstream benefits and resources available to low-income residents   

- Skills in: 
Microsoft Office  

Selection Plan

Applicants who meet the minimum qualifications will be evaluated on the education and work experience in their employment profiles and the responses to their supplemental questions.  The highest scoring applicants, but no more than the top 20 will proceed to the resume review evaluation. 

Following the resume review evaluation process, the highest scoring applicants, but no more than the top 10 will be invited to an oral board panel panel interview.

  • Oral Board Panel Interviews will be held the week of May 31, 2022

Upon completion of all examination processes, the highest scoring applicants will be placed on the Civil Service Employment List.  An applicant’s ranking on the Civil Service Employment List will be based on their final score which will be calculated as:

  • 10% of the rating for education and work experience 
  • 10% of the rating for the Resume Review
  • 80% rating for the Oral Board Interview

This recruitment will establish a civil service list that may be utilized to fill vacancies occurring in this classification within the next 6 months. Post list hiring interviews may be conducted prior to final selection.
