PATTERN I

Do you have one year of experience as an Information Systems Specialist II (or equivalent class) in San Joaquin County?

PATTERN II

Have you completed at least 60 semester (or 90 quarter) units from an accredited college or university, with at least 24 units in computer science?

If you answered "Yes" to Question No. 2, please provide:

*  The name of the college/university you attended;
*  The number of total units you have completed; and
*  List the course title and number of units for each computer science course you have completed.

Information Systems Technology Certification (may substitute for all or part of the required education).

Please provide a listing of all of the Information Systems Technology certificates or training that you have completed.

Do you possess three years of full-time, paid experience that included substantial responsibility for operating, installing, configuring, testing, troubleshooting and repairing desktop computer hardware, software, peripheral equipment and/or network systems?

If you answererd "Yes" to Question No. 3, provide a detailed description of the duties you performed. Please include the name of your employer(s), dates of employment and the number of hours worked per week performing those duties.
