Shifts:  Varied.  Days, evenings, nights, overnights, weekends, and/or holiday shifts may be required, depending on position. The hospitals operate on a 24-hour, 7 day per week basis and some Department of Public Health centers/clinics are open evenings, nights, weekends, and/or holidays.

EPIC Electronic Health Record (EHR) Project:

The San Francisco Department of Public Health is in the midst of a project that will shape the very way that we provide patient care.  Epic will bring our entire network of care closer together and allows us to provide better patient care, improve patient experience and provide more cost-effective care.  The EHR provides staff with a better integrated tool to accomplish their work and will bring our entire network of care closer together.  Our new Epic EHR will improve the patient experience for DPH’s more than 100,000 patients per year, who visit us more than 550,000 times annually.  It is an essential tool for the organization to function as a unified network so that patient care and experience is consistent and of the highest quality, wherever the patient may be. The unified EHR will support 380,000+ patients/clients annually.  The new system’s “Go-Live” date is August 3, 2019.

General Job Description:  These 2903 Hospital Eligibility Worker positions will be integral as we embark on this Epic transformation into the modern era of health care.  These positions will assist the practice/department in delivering the highest quality care, meeting and exceeding patient satisfaction and financial and operational targets.  Schegistrars schedule, register, and determine patient eligibility for various programs for seamless patient experience. Assists with prioritization and time management to ensure efficient workflow.

Under general supervision, the Hospital Eligibility Worker performs various technical duties in the screening, determination of patient eligibility and enrollment for medical coverage under the terms of various private and public health care and financial assistance programs.  The essential job functions include the following:

  • Interviews patients, their relatives and others to obtain financial and demographic information and determine eligibility for coverage of medical care under various Federal, State, or County programs or under the provisions of private or public health care plans; verifies insurance and arrange for billing of the appropriate agency or health care plan; 
  • Conducts patient registration and referral; Admits and registers patients for clinic and hospital services;
  • Explains eligibility programs, qualification standards, policy and procedure to patients; assists patients in completion of applications, forms and reports when necessary; reviews and evaluates applications for completeness and accuracy;
  • Prepares patient budget to determine eligibility; 
  • Educates patients and patient representatives of the eligibility requirements, application process, and verification requirements of applicable programs;
  • Prepares records related to patient eligibility information and financial assistance received, updates records, submits required reports;
  • Identifies, assists, enrolls, and applies clients for entitlement programs and updates patient information for Family Planning, Cancer Detection Program, CHDP, MIA, ADAP, Healthy Families, Healthy Kids and Healthy San Francisco;
  • Interpret and enforce Federal, State, and local regulations in regards to eligibility determination;
  • Prepares correspondence to client/patient and or others regarding eligibility for health care programs and/or other benefits; 
  • Assesses potential eligibility linkage and refers patients to State and Federal programs;
  • Assists patients in interpreting clinic bills, consulting with billing office on patient's behalf and making direct referrals to billing office when needed;
  • Composes and prepares correspondence to patients, references, and State agencies, gathering statistical data and preparing reports; 
  • Answers questions and provides information to patient and the general public regarding assigned program area;
  • Refers incoming callers and patients to appropriate case management and nursing staff;
  • Schedules patient appointments;
  • Documents patient encounters including but not limited to verifying patient contact information, sending and receiving patient and healthcare provider messages using an electronic health record system; 
  • Answers a high volume of telephone calls, asks pertinent questions and responds verbally and electronically to patient inquiries;
  • Adhere to HIPAA and patient confidentiality requirements. 

The Hospital Eligibility Worker performs related duties as assigned/required.

Minimum Qualifications

Experience:
1.Two (2) years of direct public contact clerical work experience which must have included the following: use of computer applications to input, retrieve and analyze information; working with a diverse client population; performing responsibilities involving interviewing to acquire detailed personal or confidential information; interpreting and applying rules, regulations and policies; creating and processing documents related to financial or personal histories of clients; and assisting clients with the completion of forms or correspondence.
OR

2.One (1) year of experience determining eligibility for health and/or social services programs, loans, financial assistance, unemployment or veterans benefits.
OR

Education: 
3.Sixty (60) semester units or ninety (90) quarter units from an accredited college or university.

Substitution:

Thirty (30) semester units or forty-five (45) quarter units from an accredited college or university may substitute for one (1) year of the work experience under qualification pattern number one (1) above.

2,000 hours of experience = 1 year of experience.

Selection Procedures

The selection process will include an evaluation of applicants in relation to minimum requirements. Depending on the number of applicants, the Department may establish additional screening mechanisms to comparatively evaluate the qualifications of candidates. If this becomes necessary, only those applicants whose qualifications most closely meet the needs of the Department will be invited to move forward in the selection process. Applicants meeting the minimum requirements are not guaranteed advancement in the selection process.

Conviction History

As a finalist for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) conflicts with the specific duties and responsibilities of the job for which you are a finalist. If a conflict exists, you will be asked to present any evidence of rehabilitation that may mitigate the conflict, except when federal or state regulations bar employment in specific circumstances, such as:

  • Candidates for positions with the Unified School District and the Community College District may be disqualified from consideration should their conviction history not meet the standards established under the California Education Code.

Disaster Service Workers

Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing. Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
