APPOINTMENT TYPE: Temporary Exempt, full-time position not to exceed six months or 1,040 hours in a fiscal year. This position is excluded by the Charter from the competitive Civil Service examination process, is considered "at will" and shall serve Appointing Officer.
Who We Are
Through the provision of coordinated, compassionate, and high‐quality services, the Department of Homelessness and Supportive Housing (HSH) strives to make homelessness in San Francisco rare, brief, and one time. The Department provides assistance and support to homeless and at‐risk youth, adults and families to prevent imminent episodes of homelessness and end homelessness for people in San Francisco. Services include outreach, homelessness prevention, emergency shelter, drop‐in centers, transitional housing, supportive housing, short‐term rental subsidies, and support services to help people exit homelessness. What We Do
San Francisco is a pioneer in homeless services and a leader in providing supportive housing as a permanent exit from homelessness. The Department of Homelessness and Supportive Housing seeks to be a national leader in the movement to end homelessness by developing a coordinated, client-focused system of services, piloting innovative models, and implementing proven solutions with measurable results. Major programs include: street outreach and service connection through the Homeless Outreach Team; 1,500-bed shelter system for adults and families including shelters for members of the LGBT community and survivors of domestic violence; Navigation Centers that provide temporary shelter to chronically homeless individuals using a low-threshold entry model; rapid rehousing rental subsidies for families, adults, seniors and transitional aged youth; the Homeward Bound program which has helped 10,000 individuals return to stable housing in their city of origin; and robust supportive housing programs of nearly 7,400 units which provide permanent housing and services to formerly homeless individuals and families.
What You’ll Be Doing
HSH leads the six core components of the Homeless Response System:
Problem Solving provides opportunities to prevent people from entering the Homelessness Response System and to redirect people who can resolve their homelessness without the need for ongoing support. It may offer a range of one-time assistance, including eviction prevention, legal services, relocation programs (Homeward Bound), family reunification, mediation, move-in assistance, and flexible grants to address issues related to housing and employment.
Coordinated Entry organizes the Homelessness Response System with a common, population specific assessment, a centralized data system and “by name” database of clients, and a prioritization method. This directs clients to the appropriate resources and allows for data-driven decision making and performance-based accountability. The Coordinated Entry process is organized to serve three subpopulations: Adults, Families with Children, and Youth. The process is comprised of four parts: access, assessment, prioritization, and referral.
Street Outreach connects those living outside with the Homelessness Response System. This includes outreach and engagement (San Francisco Homeless Outreach Team), encampment resolution (Encampment Resolution Team), care coordination, Access Points, and Resource Centers.
Temporary Shelter provides temporary places for people to stay while accessing other services and seeking housing solutions. This may include shelters, Navigation Centers, Stabilization Beds, and Transitional Housing. Housing provides permanent solutions to homelessness through subsidies and housing placements. This may include time-limited supports such as Rapid Rehousing and time-flexible programs such as Rent Subsidies and Permanent Supportive Housing.
Housing Ladder offers opportunities for residents of Permanent Supportive Housing or Rapid Rehousing to move outside of the Homelessness Response System (Moving On Initiative).
Essential Duties and Responsibilities
- Conducts outreach and informational presentations at emergency shelters, homeless resource centers, supportive housing sites
- Provides other related programs and services to clients who are experiencing homelessness in San Francisco
- Applies eligibility criteria and complete intakes
- Maintains all required logs to track clients and outcomes
- Makes phone calls according to eligibility criteria
- Performs other related duties as assigned
These minimum qualifications establish the education, training, experience, special skills and/or license(s) which are required for employment in the classification. Please note, additional qualifications (i.e., special conditions) may apply to a particular position and will be stated on the exam/job announcement.
One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties: Serving as a liaison between targeted communities and healthcare agencies; providing culturally appropriate health education/information and outreach to targeted populations; providing referral and follow up services or otherwise coordinating care; providing informal counseling, social support and advocacy to targeted populations; escorting and transporting clients; providing courier/dispatcher functions; performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.
License and Certification:
Must possess a valid California Driver's License for positions requiring driving.
Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.
