Camp Mather 2019 Season

2604 Food Service Worker Supplemental Questionnaire

This Supplemental Questionnaire gathers job-specific information in order to expedite the selection process and address administrative matters. You must possess the minimum qualifications as stated in the job announcement at the time you submit your application in order to be considered for this position. You must be at least 18 years or older at the time of hire. You must be willing to live in shared housing with other camp staff. You must be able to secure your own transportation to and from Camp Mather. Please read all of the items below carefully and respond to each completely.


1.

The 2019 Camp Mather season begins April 21 and ends September 30. The season is divided into three phases: early crew, high season, and wrap-up crew. Food Service Workers work 6-days a week and staff a variety of shifts including weekends. Please select any of the following phases in which you are available to work at Camp Mather.

Please note: hiring priority is given to those who can work the early crew, high season AND wrap-up crew phases, in part or in full.

I can work the early crew from April 21 - June 1
I can work the high season from June 2 - August 17
I can work the wrap-up crew from August 18 - September 30
I can work the entire season from April 21 - September 30

If you have a unique summer schedule, please write the beginning and ending date(s) of your availability to work at Camp Mather. If you were able to fully describe your availability to work, above, please write "N/A".

Please list any vacation dates you may need between April 21 - September 30 in the box below (examples include dates for important school events or family events). please include dates even if they are tentative. This information is important as it helps us plan work schedules to accommodate your time-off requests. If you don't have any plans for time-off please write "N/A".

Camp Mather does not provide transportation to and from Camp. You are responsible for securing your own transportation. Possible transportation methods includes personal vehicle, family member driving you to and from camp, carpooling with others to camp, flights to the Bay Area if out of state, etc.  For your reference, the address to Camp Mather is: 35250 Mather Road, Groveland, California 95321.

Please indicate how you will get to Camp Mather to start your assignment and how you will get home from Camp Mather when you finish your assignment. You must respond to this question in sufficient detail.

If you are located less than a 2-hour drive from San Francisco, you are required to attend an in-person interview. However, if you are located more than a 2-hour drive from San Francisco, you may request a Skype interview. Please indicate below as appropriate.

CERTIFICATION OF AGE – If selected for the position, I certify that I am age 18 or older at the time of hire.

 

CERTIFICATION - AWARENESS OF LOCATION - I certify that I understand that Camp Mather is located in the Sierra mountains near Yosemite National Park, approximately 180 miles east of San Francisco and that I am required to find my own method of transportation to and from Camp Mather.

 

CERTIFICATION - AWARENESS OF WORK CONDITIONS - I certify that I understand that the working conditions at Camp Mather include living in shared housing, that both indoor and outdoor assignments at Camp Mather often involve exposure to high heat, bugs and insects; and that room and board is provided during employment at Camp Mather.

 

CERTIFICATION OF MINIMUM QUALIFICATIONS -  I self-certify that I am a high school graduate, OR that I have received my GED, OR that I will have graduated from high school by June 2019. I am attaching to this application documentation which validates my high school status. (Documentation, in PDF format, may comprise a high school diploma; OR a letter from your high school, signed by an official representative, which states that you will have graduated by June 2019; OR a copy of your GED; OR a transcript from your most recent semester/quarter of college attended.) AND that I have at least six (6) months of food handling experience in the food service industry as a food service worker, kitchen helper, waiter, busboy, or similar position; OR that I have completed 12 (twelve) semester units or equivalent credits in Food Service, Culinary Arts, Dietetics and/or Nutrition.

 

OVERALL CERTIFICATION - I certify that the information provided in this Supplemental Questionnaire is true and based on my training, skills and experience. I understand that any false or incorrect statement may result in my disqualification for and/or dismissal from employment with the City and County of San Francisco.
