The purpose of this Supplemental Questionnaire is to determine your minimum qualifications that have been identified as critical for satisfactory performance in positions in this class. The information provided should be consistent with the information on your application and is subject to verification.

Instructions: Question A will be used to determine if you meet the Minimum Qualifications required to apply for this 9914 position. Responses provided to Question A are not scored; however, applicants who do not possess the required Minimum Qualifications as stated on the job announcement will not be allowed to participate in the examination process.
