SUPPLEMENTAL QUESTIONNAIRE

The purpose of this supplemental questionnaire is designed to assess your experience as it relates to the knowledge, skills, and abilities linked to this 0923 Manager II, Animal Care Operations Manager position. The information you provide on this questionnaire will be used in evaluating whether you possess the experience required for this position. The supplemental questionnaire will be used to assess possession of minimum qualifications and to determine candidates’ score on the Supplemental Questionnaire Evaluation. Of interest are the knowledge, skills and abilities you have acquired through the years of experience that relate to the essential functions identified for position in this class. This information is subject to verification and must be included in your application/employment record.
