PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY AS THEY CONTAIN INFORMATION THAT MAY AFFECT YOU EXAMINATION SCORE AND RANK ON THE ELIGIBILE LIST:

The purpose of this Supplemental Questionnaire (SQ) is to evaluate your training and experience as they relate to the knowledge, skills, and abilities linked to the essential functions of this 0933 Director of Supportive Housing position (please refer to the job announcement for a more detailed description of these knowledge, skills and abilities).

Question 1 to 6 will only be used for screening purposes only.

Only the information you provide in your answers to question 7  to 10 will be evaluated to determine your score in the selection process for this position.

No attachments or additional documents such as resumes, cover letters, or employment applications will be considered (i.e. Writing ‘see resume’ or ’N/A’ is not a sufficient response).

For the questions that ask you to describe your experience, you must provide your response in narrative form by using complete sentences.  Do NOT only list or bullet your response, or copy and paste directly from your resume.  Please be thorough but concise.  It is suggested that you review the entire questionnaire before starting, prepare the narrative-style responses to some of the items in a word processing document, and then paste them into the questionnaire. A RESPONSE OF “SEE RESUME” OR “SEE APPLICATION” WILL NOT BE ACCEPTED.

The SQ will be presented to an expert review panel for an assessment and will be used as part of the examination process to determine candidates'' score and rank on the eligible list. This Supplemental Questionnaire will account for 60% of the total weight of the candidates’ final score. Insufficient or non-responsive answers to the SQ may result in disqualification from the recruitment process.
