The purpose of this Supplemental Questionnaire is to obtain specific information regarding training and experience as they relate to the knowledge, skills and abilities linked to the essential functions of this position.
The information you provide in this questionnaire should be consistent with the information in your application. All relevant experience must be listed in the Employment Record portion of your application and is subject to verification. Experience that is not listed will not be counted.
The questionnaire will be used as part of the examination process to determine candidates’ score and rank on the eligible list in conjunction with the Management Test Battery. This information will be made available to departmental personnel and management staff to assist in the hiring decisions.
For the questions that ask you to describe your experience, you must provide your response in narrative form by using complete sentences. Do NOT only list or bullet your response, or copy and paste directly from your resume. Please be thorough but concise. It is suggested that you review the entire questionnaire before starting, prepare the narrative-style responses to some of the items in a word processing document, and then paste them into the questionnaire. A RESPONSE OF “SEE RESUME” OR “SEE APPLICATION” WILL NOT BE ACCEPTED.
Be sure to answer each question completely. Each response is evaluated independently from the other questions. You may need to repeat some information from one response to another in order to best respond to a given question.
Your score received based on your responses to this Supplemental Questionnaire will account for 60% of the total weight of the candidate’s final score. Insufficient or non-responsive answers may result in disqualification from the recruitment process.
