The qualifying questionnaire (questions A - C) will be used to determine possession of minimum qualifications.  Your responses below must be consistent with the information you provide in the employment history section of the application form. Candidates must possess the minimum experience/education requirements by the final filing deadline in order to move forward in this process.  You may be asked to provide verification of your qualifying education and experience at any point in the recruitment and selection processes.

You must also completely fill out the Employment History section of the application.  Your education and experience must be described in detail on the official application in order to be considered.  Writing "see resume" will not be accepted and may disqualify you as a candidate.

SUPPLEMENTAL QUESTIONNAIRE EXAMINATION

Read carefully as it will affect your examination score and rank of the Eligible List. 

The purpose of this Supplemental Questionnaire (SQ) is to describe your training and experience as they relate to the knowledge, skills, and abilities linked to the essential functions of this 2486 position.  Only the information you provide in your SQ answers will be evaluated to determine your score in the selection process for this position. NO attachments or additional documents such as resumes, cover letters, or employment applications will be considered.

The SQ will be presented to an expert review panel for an assessment and will be used as part of the examination process to determine candidates'' score and rank on the eligible list. This Supplemental Questionnaire will account for 100% of the total weight of the candidates’ final score. Insufficient, non-responsive and/or answer that are plagiarized or falsified may result in disqualification from the recruitment process.

The responses that you provide to this questionnaire should be consistent with the information on your application, and are subject to verification.

It is suggested that you prepare your responses in a word document and then paste them into the questionnaire. Responses should be sufficiently detailed to assist in evaluating your qualifications for this position. Briefly describe your overall experience performing quantitative and qualitative analysis of water, wastewater, or other environmental elements. Pick one analysis that you conducted and describe in detail the specific Environmental Protection Agency (EPA) or other approved methods and matrices you performed to conduct the analysis. (450 words max)

Describe your overall experience and maintenance of general laboratory equipment and analytical instruments.  What safety procedures do you follow when handling the equipment?  Describe a specific situation when an instrument was malfunctioning and providing incorrect results of an analysis?  How did you safely troubleshoot the problem to get the correct result? (450 words max)

3.

Briefly describe your general knowledge and experience of Quality Assurance and Quality Control (QA/QC). Now, provide a specific example of a QA/QC failure and describe in detail the steps you took to resolve it. (450 words max)   

4.

Describe your experience interpreting and summarizing data collected from analytical instruments and chemical analysis.  Provide an example and tell us how did you calculated your results? What was concluded and how did you present your results? (450 words max)
