Responses to items on the Minimum Qualification Supplemental Questionnaire must be supported by the information provided on the application in order to receive appropriate credit.  Please provide a response to each question below to the best of your ability.  Please provide all information requested even of the information may appear redundant.   Do not write, “See application” or “See resume.”

Supplemental Questionnaire Evaluation

PLEASE READ THE FOLLOWING INSTRUCTIONS CAREFULLY AS THEY CONTAIN INFORMATION THAT MAY AFFECT YOUR EXAMINATION SCORE AND RANK ON THE ELIGIBLE LIST:

The purpose of this Supplemental Questionnaire Evaluation (SQE) is to gather information about your experience as it relates to the knowledge, skills, and abilities essential to perform the functions of the 4230 Estate Investigator – Public Conservator / Public Guardian. Candidates will have their Supplemental Questionnaire responses rated and scored. A passing score must be achieved on the Supplemental Questionnaire Evaluation in order to continue in the selection process.

YOU MUST:

1. Provide all required verification. Unverified experience, education, licensing, etc. will not be credited. Each page of the Supplemental Questionnaire will direct you as to the type(s) of verification or information that you must supply. They are:

Verification of Qualifying Experience and Education: The examination announcement contains information about the requirements for verification. Please supply as much verification as is necessary to meet the minimum requirements for this examination.

The Name of a Person Who Can Verify Information: The person should be someone such as a supervisor, program director or personnel officer who has personal knowledge that you either performed the specific  activities or that your position required you to perform such activities. You may use the same person, if appropriate. 

2. Complete each section of the application and Supplemental Questionnaire as indicated even if the information may appear redundant. Do not write, "See application."

3. Be thorough but concise. All of your information must be supplied in the spaces provided. Provide your best or highest examples of work.

You may list unpaid work experience on the Supplemental Questionnaire and regular application. You may use the same work experience or list the same employer more than once under each question or for more than one question if you feel that it is appropriate. For example, you may have held different positions or gained different work experiences while working for one employer. In that case, you might want to list your experiences separately in order to demonstrate the diversity of experiences you gained.

Your responses to the Supplemental Questionnaire will be rated by a panel of experts and will be used as part of the examination process. This Supplemental Questionnaire will account for 100% of the total weight of candidates’ final scores. Plagiarized or have falsified information may result in disqualification from the recruitment process.

Only the information you provide in your answers will be rated to determine your score. No attachments or additional documents such as resumes, cover letters, or employment applications will be considered (i.e. Writing ‘see resume’ or ’N/A’ is not a sufficient response). The responses that you provide to this Questionnaire should be consistent with the information on your application and are subject to verification.

It is suggested that you review the questions before starting, prepare your thorough narrative- style responses in a word processing document, and then paste them into the Questionnaire. Please limit responses to one page unless otherwise instructed.
