SUPPLEMENTAL QUESTIONNAIRE EXAMINATION

Read carefully as it will affect your examination score and rank of the Eligible List. 

The purpose of this Supplemental Questionnaire (SQ) is to describe your training and experience as they relate to the knowledge, skills, and abilities linked to the essential functions of this 1825 position.  Only the information you provide in your SQ answers will be evaluated to determine your score in the selection process for this position. NO attachments or additional documents such as resumes, cover letters, or employment applications will be considered.

The SQ will be presented to an expert review panel for an assessment and will be used as part of the examination process to determine candidates'' score and rank on the eligible list. This Supplemental Questionnaire will account for 100% of the total weight of the candidates’ final score. Insufficient, non-responsive and/or answer that are plagiarized or falsified may result in disqualification from the recruitment process.

This position will require managing all aspects of the SFPUC Information Technology Services’ $25 million plus annual budget. Describe in detail your experience with how you have conducted budget planning, development, monitoring, reporting and forecasting. (500 words max)

This position will require overseeing and/or processing all technology-related procurements on behalf of the SFPUC. Describe in detail your experience with respect to technology related procurements. Include a detailed summary of your experience with the issuance of Requests for Proposals, Requisitions, Purchase Orders, and Professional Service contracts and which technical system(s) you have used. (500 words max)

This position will require developing a hiring plan for SFPUC’s entire IT department.  Describe a professional experience where you needed to balance the needs of the organization/department with consideration to the budget, personnel vacancies, and other resources.  Discuss your strategies and solutions for your predicament.  (500 words max)

Describe a time when you were either leading or coordinating a team project.  What steps did you take to ensure your team was successful?  How did you ensure growth and development within the team?  (450 words max)
