Black Infant Health (BIH) Program

Appointment Type: Temporary Exempt (TEX Category 18): This position is excluded by the Charter from the competitive Civil Service examination process and shall serve at the discretion of the Appointing Officer and the duration shall not exceed 3 years.
The Black Infant Health program works to achieve State specified activities to reduce African American infant mortality through a comprehensive community based effort assuring that at-risk pregnant and parenting African American women, their infants up to age one, and their families have access to quality maternal and child health services.
Location: SF Black Infant Health, 1290 Fillmore St., San Francisco, CA 94115
Shift: Monday - Friday, 8am - 5pm; requires occasional weekends and/or evening coverage
Under the general direction of the Black Infant Health Program Director, provides outreach and community relations services and assists with coordinating and managing BIH program activities.
Facilitates prenatal and postpartum group sessions;
Develops and maintains a site-specific Recruitment Plan for BIH;
Establishes a database of community agencies and create relationships to obtain BIH referrals;
Maintains relationships with medical and community service providers who are the primary referral sources into BIH; Develops Partnership Agreements to assist in providing referrals to the BIH Program;
Conducts outreach activities on a regular basis; for example, conducting in-service trainings and BIH orientations for Partnership Organizations including but not limited to media, advertisement and campaigns;
Attends inter-agency and community meetings;
Conducts presentations to hospitals, clinics, churches and CBO’s to inform community about BIH program services; and Attends CDPH/MCAH-sponsored program meetings and job specific trainings.
The Health Worker III performs related duties as assigned/required.
Two (2) years of verifiable experience within the last five (5) years, working with a culturally diverse population performing a combination of at least two (2) of the following duties:
· Serving as a liaison between targeted communities and healthcare agencies;
· Providing culturally appropriate health education/information and outreach to targeted populations;
· Providing referral and follow up services or otherwise coordinating care;
· Providing informal counseling, social support and advocacy to targeted populations;
· Escorting and transporting clients;
· Providing courier /dispatcher functions;
· Performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.
Note: One year of experience equals to 2,000 hours.
Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice. Facial hair or any condition that interferes with a face-fitting respirator’s seal (i.e. comes between the sealing surfaces of the respirator and the wearer’s bare skin) is not permitted when face-fitting respirators are worn, including during initial or periodic respirator fit-testing.
