Appointment Type: This is a temporary exempt (TEX) appointment that may not exceed three (3) years. Exempt employees are considered 'at will' and serve at the discretion of the appointing officer.

The San Francisco Department of Public Health (DPH) is accepting applications for temporary exempt positions in class 2586 Health Worker 2 to work at the Community Health Equity and Promotion (CHEP) Branch.

The qualified applicant pool may be utilized to fill future temporary exempt vacancies in this class at other locations within DPH.  

Job Description: 
Under supervision of the Community Health Response Team Lead, the Health Worker 2 will work on the Outreach Prevention Treatment Integration Project (Project OPT-IN) and support the work of the Street Medicine Team and Care Coordinator/Navigator by engaging with drug users and the homeless as part of health events, on the street and in encampments with the goal of providing access to services. The essential duties include:

  • Engage in follow up and linkage services and assist with navigation for PrEP, HIV medication adherence, and Buprenorphine follow-up;
  • Provide syringe disposal education and supplies/materials during all outreach activities;
  • Participate in safe syringe disposal sweeps in hot-spots throughout the City;
  • Partner with people who inject drugs and other community members to ensure drug user health needs are being met;
  • Provide education, information, and referrals on harm reduction, and Narcan to community members;
  • Attend and participate in discussions on the planning and implementation of encampment health events;
  • Maintain tracking log of engagements and contacts in the community and other program data;
  • Draft and organize outreach materials;
  • Help organize and participate in workshops, meetings, outreach activities, and other events related drug user health and/or homelessness; and
  • Develop and maintain tracking log of outreach supplies and orders.

The Health Worker 2 performs other related duties as assigned/required. 

Working Conditions: Some positions may require employees to work directly with individuals within locked jails or psychiatric facilities; the homeless population and individuals who have or are carriers of infectious/communicable diseases. Some positions may also require exposure to inclement weather conditions and/or heavy lifting.

Minimum Qualifications

Experience:  One (1) year of verifiable experience within the last five (5) years, performing a combination of at least two (2) of the following duties:

  • serving as a liaison between targeted communities and healthcare agencies;
  • providing culturally appropriate health education/information and outreach to targeted populations;
  • providing referral and follow up services or otherwise coordinating care;
  • providing informal counseling, social support and advocacy to targeted populations;
  • escorting and transporting clients;
  • providing courier/dispatcher functions;
  • performing pre-clinical examinations of vital statistics, such as measuring a patient’s weight, height, temperature and blood pressure.

Substitution: Possession of a Community Health Worker Certificate from City College of San Francisco can substitute for 6 months of experience.

One year of experience equals 2,000 hours

Desirable Qualifications: The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.

  • Ability to work well independently and as part of a team.
  • Ability to work with drug users, community partners, DPH staff and other City departments.
  • Ability to organize information and maintain accurate, timely and complete records.
  • Ability to write professional reports and correspondence.
  • Ability to multi task and prioritize large caseloads, and meet deadlines.
  • Ability to identify and utilize problem solving skills.
  • Ability to develop flyers, brochures, and other materials using various computer programs.    
  • Knowledge of Harm reduction. 
  • Excellent interpersonal, verbal and written communication skills.
  • Knowledge of the Civic Center, Tenderloin and South of Market communities/neighborhood. 
  • Understanding of San Francisco’s at-risk populations for substance use, HIV, and Hepatitis C, and knowledge of culturally appropriate strategies for prevention, care, and treatment. 
  • Understanding of substance use, mental health, and homelessness.
  • Ability to take initiative.

Selection Procedures

Minimum Qualification Supplemental Questionnaire (Weight: Qualifying):

Candidates will be prompted to complete a Supplemental Questionnaire as part of the online employment application. This Minimum Qualification Supplemental Questionnaire is designed to obtain specific information regarding an applicant's experience in relation to the Minimum Qualifications for this position. Applicants must also complete the official application. It is essential that applicants provide complete information in identifying their education, experience and training. The Minimum Qualification Supplemental Questionnaire will be used to evaluate if the applicant possesses the required minimum qualifications.

The Department may establish and implement additional screening mechanisms to comparatively evaluate qualifications of candidates. 

Conviction History

As a selected candidate for a job, you will be fingerprinted, and your fingerprints will be sent to the California Department of Justice (DOJ) and the Federal Bureau of Investigation (FBI). The resulting report of your conviction history (if any) will be used to determine whether the nature of your conviction (or arrest, in limited circumstances) conflicts with the specific duties and responsibilities of the job for which you are a selected candidate. If a conflict exists, you will be asked to present any evidence of rehabilitation that may mitigate the conflict, except when federal or state regulations bar employment in specific circumstances, such as:

Disaster Service Workers

All City and County of San Francisco employees are designated Disaster Service Workers through state and local law (California Government Code Section 3100-3109). Employees who choose not to shave and do not have either American Disabilities Act (ADA) or Equal Employment Opportunity (EEO) Accommodations do not have the right to alternate work assignments or the option of using a loose-fitting Powered Air Purifying Respirator (PAPR) in place of a FaceFitting Respirator.
