2454 CLINICAL PHARMACIST

GENERAL JOB DESCRIPTION:
Under general direction, the Clinical Pharmacist performs professional pharmaceutical duties in drug education and utilization, counsels patients on drug usage, educates health care personnel on drug utilization, and may compound drugs and dispense medicines in a clinical pharmacy. The essential functions include:
- Interviews and examines patients for drug evaluation; checks on drug compliance and side-effects; discusses symptoms and effects of prescribed drugs with patients; and maintains a drug history on patients interviewed.
- Consults with physicians on drug usage and the drug regime of specific patients particularly hypertensive, diabetic and psychiatric patients; recommends changes in the drug or the dosage prescribed for these patients.
- Conducts both formal and informal training for nursing staff, medical and pharmacy students and residents and other health care staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy, and toxicity, etc.
- Participates in the development and revision of medication use policies and guidelines.
- Compounds drugs, prepares and dispenses medications, checks pharmacy stock, and keeps records.
The 2454 Clinical Pharmacist also performs other related duties as assigned.
Nature of positions: Positions are located throughout the Department of Public Health, including Zuckerberg San Francisco General Hospital and Trauma Center (ZSFG), Laguna Honda Hospital and Rehabilitation Center (LHH), Community Primary Care and Mental Health Clinics, as well as San Francisco County Jails. Some positions may require employees to work directly with individuals within locked jails and psychiatric facilities, the homeless population and/or individuals who have or are carriers of infectious/communicable diseases. Additionally, part-time, evenings, nights, weekends, rotating and/or holiday shifts may be required.
- Possession of a valid Registered Pharmacist License issued by the California State Board of Pharmacy; AND
- One (1) of the following:
A. Completion of six (6) months of a pharmacy residency program; OR
B. Completion of six (6) months of a post-graduate pharmacy fellowship program; OR
C. Two (2) years of professional level experience as a Clinical Pharmacist (equivalent to class 2454 Clinical Pharmacist within the City and County of San Francisco); OR
D. Possession of a Board Certification in a pharmacy specialty area.
Applicants must meet the minimum qualification requirement by the final filing date unless otherwise noted.
Possible Special Conditions (some positions may require the following):
- Completion of a PGY2 Pharmacy Residency program in emergency medicine, critical care or toxicology; OR two years of full-time professional level experience as a clinical pharmacist in an Emergency Department (ED) setting.
- Bilingual fluency in a variety of languages depending on the department’s needs. Only those eligibles that pass the bilingual proficiency exam will be considered for bilingual positions.
- The ability to obtain and maintain security clearance with the San Francisco Sheriff’s Department.
The stated desirable qualifications may be used to identify job finalists at the end of the selection process when candidates are referred for hiring.
Acute Care practice:
- Demonstrated experience with the acute care medicine patient population.
- Excellent oral and written communication and interpersonal skills.
Ambulatory Care practice:
- Completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship.
- Fluency in Spanish and/or Cantonese is highly desired.
- Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network.
Behavioral Health practice:
- Completion of a psychiatric pharmacy residency.
- Clinical experience working in substance abuse or mental health settings.
- Fluency in Spanish and/or Cantonese is highly desired.
- Board Certification as a Psychiatric Pharmacist.
- Possession of a valid Advanced Practice Pharmacist license in California.
Long Term Care practice:
- Completion of a geriatric pharmacy residency.
- Certification in Geriatric Pharmacy.

Training and Experience Evaluation (Weight: 100%):
Candidates will be prompted to complete a supplemental questionnaire as part of the online employment application. The Supplemental Questionnaire is designed to measure knowledge, skills and/or abilities in job-related areas that have been identified as critical for satisfactory performance in this position.
All relevant experience, education and/or training must be on the application in order to be reviewed in the scoring process. Candidate responses on the Supplemental Questionnaire cannot be changed once submitted. Candidates will be placed on the eligible list in rank order according to their final score on the Supplemental Questionnaire. The department may administer additional position-specific selection procedures to make final hiring decisions.
Certification Rule: The certification rule used for the eligible list resulting from this selection procedure will be Rule of Ten Scores. Additional position-specific selection mechanisms may be conducted by the hiring department prior to making final hiring decisions.

The duration of the eligible list resulting from this examination process will be six (6) months, and may be extended with the approval of the Human Resources Director.

Disaster Service Workers

Note on Personal Protective Equipment (PPE): Some positions in the Department of Public Health will require the use of personal protective equipment (PPE), including but not limited to gloves, gowns, eye and face protection, and face-fitting respirators. The requirement for the use of PPE may come on short or no notice.
