Note: The supplemental questionnaire and the application are administered as two separate documents. Panel members assigned to review and score the written responses to the supplemental questionnaire will ONLY have access to these responses. They will NOT have access to any information contained in the application or resume.

It is recommended that you respond to all questions with concise but detailed answers and provide all requested information in each response. Each answer will be scored separately on its own merit.

Describe your experience assisting users of a particular systems’ software application where detailed knowledge of the specific application is required. In your response, include the name of the application(s), the types of application(s) issues with which you assisted users, and the communication style(s) used.

2.

Describe your experience recording and maintaining documentation of reported application software issues or problems. In your response, include any software used for tracking, as well as the methods and reasons used for documenting these issues.

3.

Describe your experience performing routine application support duties such as monitoring or adding applications, users, and devices; modifying user profiles; resetting passwords; and performing file maintenance.

4.

Describe your experience developing and presenting training materials in a classroom, small group, or one-on-one training setting related to new and/or modified application modules (i.e.: process mapping, information newsletters, procedural memos, voicemail usage, electronic mail usage, and/or internal process development).

5.

Describe your experience troubleshooting, debugging, and resolving a variety of operational and/or business-related application problems.

6.

Describe your experience supporting a statewide system and assisting in the evaluation of upcoming changes to the software application. In your response, include how you assisted in identifying impact on current systems and/or processes, provided input on necessary revisions, and assisted with implementing those changes.
