SoonerQuit Coordinator

POSITION REQUIREMENTS:
This position reports to the SoonerQuit Supervisor and is responsible for working collaboratively with key state agencies to research, develop, promote, review, and revise focused initiatives to advance and improve tobacco cessation, diabetes management/prevention efforts and to nutrition efforts within the SoonerCare population and healthcare providers, community leaders and among invested stakeholders. Engage, educate, and promote tobacco cessation, diabetes management/prevention, nutrition best practices to healthcare providers, community partners, invested stakeholders and members. Develop and promote SoonerCare benefits, educational, and communication resources related to SoonerQuit initiatives. Actively work to identify opportunities to develop partnerships for related initiatives, with the ultimate goal of statewide penetration.

PRINCIPLE ACTIVITIES: 
The principal activities include the following:
- Identify Healthcare practices/providers statewide to participate in the SoonerQuit program, to educate providers on and assist with the implementation of SoonerCare tobacco cessation, diabetes management/prevention, and nutrition best practices.
- Conduct programmatic outreach to SoonerCare providers statewide to engage in the program. Implements action plan for provider education related to tobacco cessation for providers participating in initiative.
- Initiate and maintain contact with providers and partners through a variety of methods including in-person and phone contacts, email, and written correspondence.  Provide continued education and technical assistance related to implementation and sustainability of best practices.  Maintain current initiatives and identify opportunities to develop additional SoonerQuit projects in alignment with TSET goals and objectives to engage, educate and promote tobacco cessation, diabetes management/prevention, and nutrition best practices among providers, partners, and members. 
- Coordinate outreach, promotion, and enhancement of the SoonerQuit initiatives and other related projects. Maintain and develop SoonerQuit partnerships and collaborations, engage community partners and members, actively participate in and seek out meetings/workgroups that would further aid in accomplishing objectives set forth in the annual work plan.
- Increase awareness, reduce barriers, and increase utilization of services t through outreach efforts and with the development of communication materials and dissemination through various media avenues that include websites, newsletters, social media, community coalitions and vendor coordinated efforts. Collect and report activities to the SoonerQuit Supervisor and program evaluation vendor.  Contribute data required for the external evaluation. Analyze formal reports for trends and gaps. Recommend process revisions to improve the efficiency and effectiveness of the SoonerQuit program based on observations and feedback. 
- Stays informed of current research, updates and strategies for best practices related to tobacco cessation, nutrition, and physical activity.
- Ensures administrative procedures and applications to track grant required information are established and followed.
- Represents OHCA as needed in meetings, events, seminars, and training forums.
- Other duties as assigned.

TRAVEL: Travel on an “as needed” basis.

MINIMUM QUALIFICATIONS

KNOWLEDGE AND BACKGROUND REQUIREMENTS
QUALIFICATIONS
: To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. 
The requirements listed below are representative of the knowledge, skill, and/or ability required. 

EDUCATION AND/OR EXPERIENCE:
- Bachelor’s degree in Public Health or a health care related field AND
- 1 year of experience with provider outreach programs, tobacco prevention/cessation programs, or work experience in a provider setting

PREFERENCE MAY BE GIVEN TO CANDIDATES WITH:
- Advanced Education in a health care related field
o SoonerCare or Medicaid experience
- Experience with health promotion campaigns
- Experience with development and management of digital communication str.
