Main Purpose of Job

The main purpose of this full-time position will be to provide care coordination services and program oversight for the Community Consortium Hub Program serving school age children. The Community Supports Coordinator will oversee and assist with provider/spokes to assure they are meeting the requirements of their conditions of award. This position will also coordinate service providers/spokes.

This position will work with the Program Manger to assure fiduciary responsibilities by managing grants from the CHRC (approximately $400,000 budget) and awarding grants to Spokes as subgrantees, and collecting and reporting data and engaging in a statewide quality improvement learning collaborative. The Coordinator will provide care coordinators services as assigned. This position will also support and collaborate with local providers and agency partners to ensure services are meeting the needs of the community. In addition, this position will provide program updates, grant monitoring/reporting, oversight for critical and caseload oversight.

MINIMUM QUALIFICATIONS

Education:   Graduation from an accredited high school or possession of a high school equivalency certificate.

Experience: Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources.

Notes:

1. Candidates may substitute 30 college credit hours from an accredited college or university on a year-for-year basis for the required experience.

2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should possess:
  • Experience working with children, adolescent and young adults with behavioral health.
  • Experience working with community partners to include completing needs assessments.
