Main Purpose of Job

The Transition Coordinator assists Maryland's School for the Deaf students and their families in obtaining services necessary to transition from middle school to high school and to post-secondary setting, whether for post-secondary education, employment or long-term support.

POSITION DUTIES

· Develop, coordinate and implement transition plans with in the IEP

· Provide age appropriate transition assessment to students to support development of post-secondary goals in areas of employment, education, training or independent living goals.

· Meet with students to discuss courses as they relate to their postsecondary goals

· Provide opportunities to meet with college recruiters or recruiters from vocational training programs

· Develop, provide and monitor transition activities in areas related to academic, employment, transportation, activities of daily living and independent living skills.

· Develop and sustain collaborative relationship with adult agencies and facilitates referrals with adult service providers

· Disseminates information to staff, students and parents regarding student needs, progress and resources

· Work closely with Work to Learn staff who provide Pre- Employment Transition Services to students

MINIMUM QUALIFICATIONS

Education:  Master’s degree in counseling, social work, education or a related field required.

Experience: Two years of satisfactory performance as a teacher or a counselor

DESIRED OR PREFERRED QUALIFICATIONS

Knowledge of the Deaf culture, Deaf Education required

Must be fluent in ASL and written English.

Experience with the Transition Process. 

Experiences working with Adult Agencies.

Demonstrate Knowledge, Skills and Abilities to provide Transition Services.

SPECIAL REQUIREMENTS

New hires are subject to a criminal background investigation at their own expense. (Maryland Code, Family Law Article 1, Sections 5-560-568)

BENEFITS

Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.

Leave may be granted to a contractual employee who has worked 120 days in a 12 month period. This leave accrues at a rate of one hour for every 30 hours worked, not to exceed 40 hours per calendar year.
