Main Purpose of Job

This position is responsible for providing Rapid Anti-HCV Antibody testing, evaluation, linkage to care, surveillance and reporting; Medical Case Management to Individuals living with HIV/AIDS or newly diagnosed individuals with HIV so that these clients can have timely access to comprehensive medical care and social services. The goal is to prevent disease transmission, delay disease progression, promote and support client independence and self-sufficiency.

May require work during day and weekend/evening assigned shifts, required to carry a smart phone.

This position is eligible for hybrid telework.

POSITION DUTIES

Clinical and Case Management (80%) 
  • Provides Rapid Hepatitis C testing to the at-risk population and address the necessary follow-up, reporting, and linkage to care for those with positive Hepatitis C RNA result.
  • Ensures that Hepatitis C case investigations are initiated and completed using Maryland Department of Health protocols, forms and NEDSS data base.
  • Provides nursing care to individuals and families living with HIV/Aids, and other clients seeking HCHD services, as needed; responsible for improvement of care and health outcomes of patients and enhancing their adherence to care thereby reducing their HIV symptoms and co- morbidities.
  • Must have the ability to draw blood, assess, evaluate and administer injections to adults and small children; participates in HIV testing clinics and communicates positive results with clients and members of the health team regarding referrals for medical services and psychosocial needs of clients; responsible for improvement of care and health outcomes of patients, and enhancing their adherence to care thereby reducing their HIV symptoms and co- morbidities; assist clients with linkage to care and referrals to medical and dental providers. 
  • Maintains regular contacts with all clients and completes needs assessments and care plans, with a goal of client self-sufficiency; responsible for client empowerment to develop and utilize independent living skills and strategies. 
  • Participates in Collaboration on special projects in the community to provide HIV/Other Sexually Transmitted Disease (STD) education and, counseling and testing. 
  • Participates in community planning and needs assessment related to HIV Program services.
  • Represents the health department as needed at various meetings related to HIV Programs and in community activities related to HIV/STD prevention and HIV testing and counseling; participates in outreach and community-based activities of the HIV team. 
  • Serves as a point of contact for Ryan White B (RWB) program; identifies, requests and maintains RWB documentation for clients; facilitates new client intake process and recertification; retrieves, reviews, and maintains updated medical records for active clients, and prepare forms and recodes for clinic; Process client’s requests for transportation/ taxi for appointments; record and maintain transportation tracking data base and log; 
  • Process client’s requests for Emergency Food Assistance/EFA; record and maintain EFA tracking data base and log.
  • Serve as liaison between the public and the Health Department regarding STI testing, treatment, and linkage to care/LTC.
  • Work synergistically with home based and regional Disease Intervention Specialists/DIS regarding care, treatment, and LTC services.

Quality Assurance and Reporting (10%)
  • Collects and analyzes RWB data to support planning and program development
  • Participates in internal meetings, chart review process, audits and case study presentations to monitor services and ensure standards of best practice
  • Maintain accurate and timely data management into CareWare/ CW for all clients
  • Attends professional development related to assigned duties;
  • Complies with standards for infection control and risk management.
  • Maintains compliance with standards of care through training including, but not limited to:  HIV/AIDS Program updates, CPR certification, Local Health Department (LHD) Nurse Dispensing, Laboratory Operations, Personal Protective Equipment, After-Hours On-Call procedures, FEMA trainings, and Data Security and Confidentiality compliance. 
  • Performs record review of hardcopy and electronic versions of NEDSS to review and correct records 
  • Provides timely and accurate statistics for Health Stat reporting
  • Participates and prepares for CD site reviews by the State Health Department and implements corrective actions.
  • Maintains patient confidentiality as appropriate in the reporting of statistics for surveillance and mitigation 
  • Attends work during assigned work hours and demonstrates a commitment to the Agency

Communication (5%)
  • Ensures confidentiality of written, electronic and verbal communication according to HIPPA.
  • Uses appropriate lines of communication within and outside the agency;
  • Responds to requests for information in a timely manner;
  • Provides interpretation and translation services according to agency policy;
  • Provides education appropriate to meet the needs of those with functional needs; 
  • Documents work accurately and within agency guidelines.

Other (5%)
  • Participates in emergency preparedness drills, exercises and after-action hot washes  
  • Accepts other responsibilities for special projects as assigned.
  • Participate in annual flu clinics as needed. 
  • Completes HCHD trainings as required. 
  • Provide coverage for weekly HIV testing clinics
  • Participates in the nurse on-call rotation for afterhours as needed.
  • Participate in community HIV and Hepatitis outreach testing activities

MINIMUM QUALIFICATIONS

Education:   A Bachelor’s degree in nursing from an accredited college or university.

Experience: None.

Notes:

1. Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a Registered Nurse for the required education.

2. Candidates may substitute two years of experience as a Registered Nurse for the required education.

3. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Nursing classification or Nursing or Health Care Specialist specialty codes in the Nursing field of work on a year-for-year basis for the required education.  

DESIRED OR PREFERRED QUALIFICATIONS

Desired qualifications:
- Experience with case management, and working with individuals from a variety of socioeconomic groups.
- Experience working with the HIV/AIDS community.
- Knowledge of community resources, HIV disease and treatment.
- Competence in Cultural and linguistic Diversity.
- Valid driver's license and ability to travel to different sites within Howard County.
- Ability to work some weekend/evenings 

LICENSES, REGISTRATIONS AND CERTIFICATIONS

1. Candidates for positions in this classification must possess a current license as a Registered Nurse from the Maryland State Board of Nursing, 4140 Patterson Avenue, Baltimore, Maryland 21215. A copy of your license or license number should be uploaded on your application.

2. Employees in this classification may be assigned duties which require the operation of a motor vehicle. Employees assigned such duties will be required to possess a motor vehicle operator’s license valid in the State of Maryland.
