MDH/Cecil County Health Department, Developmental Disabilities Program, Elkton, Maryland

Main Purpose of Job

This position will provide targeted case management services and coordination of community services to residents of Cecil County who have developmental disabilities and are, therefore, eligible for such services. For eligible individuals, this position will provide referrals for services and facilitate access to day/residential/support services. This position will also facilitate the development of the annual Person-Centered Plan, monitor individuals both in-service and on the Waiting List, and assist in transition planning. These efforts will work to maintain the individual(s) in the community, as well as to promote individual choice and overall quality of life.

MINIMUM QUALIFICATIONS

Education: Possession of a Bachelor’s degree from an accredited college or university in Nursing, Social Work, Psychology, Education, Counseling, or a related field.

Experience: Two years of professional experience in health services, one year of which must have been in professional work related to the support services and programs for developmentally disabled clients.

Notes: 1. Applicants may substitute a Master’s degree from an accredited college or university in a health or human service field for one year of the required general experience.

2. Experience in the option must be as follows: professional work related to the support services and programs for developmentally disabled clients.

3. A Bachelor’s degree from an accredited college or university in another field, plus one additional year of professional experience in health services, may be substituted for the specific degree.

4. Candidates may substitute U.S. Armed Forces military service experience as a Commissioned Officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work, on a year-for-year basis, for the required experience and education.

DESIRED OR PREFERRED QUALIFICATIONS

The desired candidate should have experience with case management, as well as experience working with children and/or adults who have a developmental disability.
