Main Purpose of Job

The Maryland Primary Care Program (MDPCP) is an essential part of the overall transformation of health care delivery in Maryland.  In order to successfully implement this change, the Program Management Office is hiring a team of individuals to provide “hands on” assistance to practices across the State.

 

Each practice coach will be responsible for developing a productive relationship with a set group of participating practices.  Coaches will provide technical assistance to program participants, serving as a key resource in the practice’s transformation journey.

 

  • Report weekly progress to Data and Practice Transformation Manager
  • Use Salesforce to track work and communicate with practices
  • Develop plans and strategy for all accounts
  • Develop strong working relationships with practice clinical and operational leaders built on trust and reliability
  • Participate in regular meetings with PMO Practices Transformation team to discuss status and recommend improvements
  • Ensure substantive, quality educational materials and technical assistance are provided to MDPCP participant practices
  • Maintain effective relationships with all program participants
  • Coordinate technical assistance with CMS’ Learning System

 

The State of Maryland entered into a Total Cost of Care All-Payer Model contract with the Federal Government beginning in January 2019 that is designed to coordinate care for patients across both hospital and non-hospital settings, improve health outcomes, and constrain the growth of health care costs in Maryland. A key element of the model is the development of the Maryland Primary Care Program (MDPCP) The MDPCP is a voluntary program open to all eligible Maryland primary care practices designed to support the delivery of advanced primary care throughout the state. The MDPCP will support the overall health care transformation in Maryland and allow primary care practices to play a more integrated and powerful role in prevention, management of chronic disease, and assuring care is provided in the most efficient and effective setting in the community.

MINIMUM QUALIFICATIONS

The position requires a bachelor’s degree from an accredited University and the ability to travel (owned vehicle). Individual should have own cell phone for work in the field. Each person will receive a dedicated state phone number for sharing with practices – calls will be forwarded to cell number.  

The ideal candidate will have 5+ years’ experience in an external, customer-facing position. Candidate should be mission-driven. Candidate should have some health care background in directly working with community providers in a consulting or employee relationship providing expertise on provider communications, networks, communications, legislation, policy, data, HIT or other related topics. Candidate should be highly knowledgeable about health care transformation, value-based payment, primary care, health and hospital systems, and trends in the delivery of health care, including how it applies to Maryland primary care providers. Candidate should be familiar with the landscape of providers in Maryland including primary care practices, hospitals, and Accountable Care Organizations.

Candidates may remain on the certified eligible list for a period of at least one year. The resulting certified eligible list for this recruitment may be used for similar positions in this or other State agencies.

BENEFITS

Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.

Leave may be granted to a contractual employee who has worked 120 days in a 12 month period. This leave accrues at a rate of one hour for every 30 hours worked, not to exceed 40 hours per calendar year.
