We're currently seeking qualified applicants for a full-time Elder Resources Case Manager. Under the general direction of the designated supervisor, manages cases of older adults needing assistance. The Case Manager is instrumental in helping older adults lead healthy and independent lives. The position coordinates, assesses, monitors, and refers clients and families to services in order to ensure that the client's needs are addressed and that they receive the necessary assistance.
A successful candidate would have a strong background in working with the older adult population, knowledge of community resources in Loudoun County, and the ability to communicate ideas clearly, concisely, and effectively both orally and in writing while working independently in the community. PRCS serves a diverse population including but not limited to youth, adults, individuals with disabilities, and seniors.

Salary commensurate with experience
Bachelor's degree in Social Work, Human Services, Psychology, or related field plus 3 years or more of experience working with older adults.
