ELDER RESOURCES CASE MANAGER

We're currently seeking qualified applicants for a full-time Elder Resources Case Manager. Under the general direction of the designated supervisor, manages cases of older adults needing assistance. The Case Manager is instrumental in helping older adults lead healthy and independent lives. The position coordinates, assesses, monitors, and refers clients and families to services in order to ensure that the client's needs are addressed and that they receive the necessary assistance.

A successful candidate would have a strong background in working with the older adult population, knowledge of community resources in Loudoun County, and the ability to communicate ideas clearly, concisely, and effectively both orally and in writing while working independently in the community. PRCS serves a diverse population including but not limited to youth, adults, individuals with disabilities, and seniors.

We are currently hiring at the following location: 

• PRCS, 742 Miller DR, Leesburg, VA

Responsibilities include, but are not limited to: 

• Plans and implements case management. 
• Meets with clients in their homes or at County sites. 
• Utilizes the Virginia Uniform Assessment Instrument (UAI) to determine eligibility for care coordination. 
• Make appropriate referrals to meet client needs and arrange service provisions. 
• Ensures client is involved in care planning allowing clients and families to take an active role in determining their needs. 
• May perform other essential job functions specific to the position and department assignment.

*Work schedule is Monday-Friday 8:30 am-5:00 pm*

Salary commensurate with experience

Bachelor's degree in Social Work, Human Services, Psychology, or related field plus 3 years or more of experience working with older adults.

PREFERRED QUALIFICATIONS: 

• Knowledge of federal, state, and local regulations governing elder services. 
• Knowledge of complexities and services available for individuals with cognitive challenges. 
• Knowledge of care coordination, case management, and income assessments. 
• Skills in delivering customer service and computerized case documentation. 
• Experience in facilitating support groups. 
• Master's degree preferred.

• Pass pre-employment criminal, credit, and DMV background checks. 
• Valid driver's license and good driving record (driving records may be reviewed annually for continued qualification). 
• Pass TB test within 30 days of hire.
