The Department of Mental Health, Substance Abuse and Developmental Services is currently seeking a Support Coordinator with the Developmental Services Division.
This position is responsible for assessing, coordinating, monitoring, and linking service delivery for children and/or adults with intellectual and developmental disabilities and their families. Experience with the HCBS waivers, CCC Plus Waiver, and Medicaid is preferred. Additionally, knowledge of community-based resources including integrated housing options and employment first principles is desired.
Duties may include assessing eligibility and needs of the individuals, including risk; interviewing individuals and their families; facilitating team meetings, coordinate service delivery among providers, developing and documenting person centered plans using a strengths-based approach. Assuring quality service provision; maintaining compliance with all State of Virginia Licensure standards, Medicaid standards, and crisis intervention and prevention. Effective oral and written communication skills, strong organizational and time management skills, proficiency in utilizing an electronic health records and other software programs for documenting service delivery is required.
The Support Coordinator is responsible for providing high-quality customer service consistent with the mission, vision, and values of MHSADS.
Hiring salary is commensurate with experience
bachelor's degree in psychology, social work, or a related human service field AND (2) years job related experience
PREFERRED:
Employment is contingent upon successfully passing pre-employment background check to include fingerprinting, credit, NCIC, DMV and TB test. Must have a good driving record and a valid driver's license in Virginia or regional state of residence.
