Introduction

Loudoun County's Department of Family Services is seeking qualified applicants with a demonstrated interest in working with older adults and adults with disabilities to join our dynamic team of Adult and Aging Family Services Specialists.

The Adult and Aging Services team provides adult protective services (APS) and case management services (AS). Each team member plays a crucial role in achieving our mission to stop adult or elder abuse, neglect, and exploitation.

 

Job Description

The successful candidate will join a diverse department that offers the opportunity to work in an energetic, challenging environment where in-depth assessment and service planning ensure the highest quality service delivery to vulnerable individuals.

The position requires an enthusiastic, diligent, and passionate team player with the capacities to initiate and complete investigations of adult or elder abuse, neglect or exploitation under difficult and potentially hostile or crisis conditions, manage competing priorities and deadlines well, collaborates with other professionals and communicate effectively orally and in writing. 

Main responsibilities for the incumbent include:

  • Completing timely abuse, neglect, and exploitation investigations involving individuals 60 years of age or older and 18 years or older and incapacitated;
  • Assessing the need for guardianship/conservatorship and facilitates the process to include medical documentation, preparation of information required for petitions, testimony in court and advocacy;
  • Assessing needs such as mental health and health interventions;
  • Conducting Medicaid Long-Term Services and Supports (LTSS) screenings;
  • Performing case management duties;
  • Documenting case contacts, service plans, and outcomes;
  • Collaborating and networking with community service providers to ensure a comprehensive system of care; and
  • Other tasks as assigned.

This vacancy may be used to fill multiple positions and will be located in our Leesburg/ Sterling office, with assigned work from other locations, as needed. Hours are 8:30 a.m. - 5:00 p.m., Mondays through Fridays, with flexible scheduling to meet operational needs.

Job Requirements

  • Requires a combination of education and experience equivalent to a Bachelor's degree in Social Work, Psychology, Sociology, Gerontology or a related field and 3 years of related experience working with older adults and/or adults with disabilities;
  • Valid resident driver's license

Must be available for overnight/weekend APS coverage (one week every 4-6 weeks) as well as to cover daytime APS intake, as needed.

Special Requirements

Preferred Qualifications

  • Master's Degree
  • APS/AS case management experience
  • Homelessness case management experience
  • Spanish fluency
