Summary Statement
This employee is involved in performing social service work requiring client/worker interaction for the Office of the Public Guardian and is responsible for interviewing and soliciting information, assessing clients needs, filling out appropriate forms, and documenting all pertinent information to ensure intake and eligibility requirements are met and service provisions are processed in an accurate and timely manner.
Essential Functions
Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. - Interviews clients, reviews cases and compiles appropriate information in order to complete all necessary forms and data sheets. Maintains supportive contact with patients, clients, and families.
- Gain and maintain contact with Federal and State agencies providing benefits and support services to clients of the agency, assisting the clients in both obtaining and maintaining benefits related to health and financial services.
- Maintains, logs, documents and records informational records and statistical reports. Maintains appropriate financial and health information on clients.
- Answers inquiries and provides information to agencies, clients, and other appropriate sources.
- May conduct follow-up by verifying information related to client and client benefits through phone contact, personal interview, and/or computer screening with family member and/or State and Federal Agencies.
- Prepares appropriate information, makes recommendations and assists in processing cases for completion, maintenance, resolution and service provision.
- With assistance, visit client homes, medical providers, hospitals, and banks to inventory and secure client property and determine appropriate disposition.
- Maintain and report client employment records where appropriate.
- Documents information in specialized case management system.
Job Requirements
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
- Six months’ experience in health or human services support such as interviewing clients and assessing personal, health, social, or financial needs in accordance with program requirements or coordinating with community resources to obtain client services.
- Six months’ experience in record keeping.
- Six months’ experience in applying laws, rules, regulations, standards, policies, and procedures.
- Six months’ experience in using an automated information system to enter, update, modify, delete, retrieve/inquire, and report on data.
- Ability to communicate courteously and effectively, both verbally and in writing.
Conditions of Hire
- A satisfactory criminal background check is required as a condition of hire.
- Direct deposit of paychecks is required as a condition of hire.
