Summary Statement

The incumbent provides support services and record keeping for staff and PROMISE beneficiaries.  The incumbent may use multiple tools, including assessment instruments approved by DSAMH, in conjunction with comprehensive assessments and other clinical information, to support individualized recovery plans. Field work may be required. The candidate will also provide support for care managers, back-up for front office staff and beneficiary assistance as assigned.

Essential Functions

  • Acts as advocate for clients with other agencies in areas such as housing, employment, clinic appointments and social, economic and community resources.
     
  • Instructs, assists and counsels clients in developing and implementing skills to allow them to function independently.
     
  • Serves as member of interdisciplinary team, to implement, recommend and evaluate comprehensive treatment for client.  Provide client counseling to assist them in achieving the highest possible level of community adjustment.
     
  • Works with clients to maintain facility, when appropriate, through maintenance and minor repairs.
     
  • Conducts classes, teaching basic principles of social interaction.
     
  • Maintains documentation of patient contacts and provide periodic reports for appropriate medical and psychological transactions.
     
  • May supervise lower level staff in the absence of supervisor.

Job Requirements

JOB REQUIREMENTS for Habilitation/Rehabilitation Specialist II
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Six months experience in health or human services support such as interviewing clients and assessing personal, health, social or financial needs in accordance with program requirements or coordinating with community resources to obtain client services.
  2. Six months experience in applying laws, rules, regulations, standards, policies and procedures.
  3. Six months experience in record keeping.
  4. Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data. 

Additional Posting Information

Due to a new requirement from the Centers for Medicare & Medicaid Services (CMS) the following mandate is effective January 27, 2022: As a condition of employment, employees providing health care services and/or support services in covered facilities must show proof that they have received the dose of a single J&J dose or the 1st dose of Moderna/Pfizer two-dose vaccine effective date January 27, 2022, and proof of the single J&J dose or the 2nd dose of Moderna/Pfizer two-dose vaccine by February. 28, 2022. Employees may not provide any care, treatment, or other services until proof is provided.  Based on CMS guidance, the date for compliance may extend into March due to the health care facilities high vaccination rate of staff.  If you accept a position with the facility, your Human Resources representative can provide more guidance.  The only exception to these requirements is for those employees granted medical or religious exemptions from the COVID-19 vaccine as recommended by the Centers for Disease Control and Prevention, which may require weekly COVID-19 testing.
