Summary Statement

The incumbent in this position will conduct intensive health and social assessments to clients in their homes. The incumbent will provide case management for clients with Alzheimer’s and related dementias, along with their families, and caregivers; facilitate in-house services and care plan determination for community programs; and use critical thinking skills to develop and implement plans designed to meet the personal, social, and health care needs of the client. Other duties include: regular contact and coordination with clients, families, physicians, healthcare facilities and other administrative staff.

  • Provides or evaluates direct nursing care.
     
  • Develops, implements and/or reviews nursing care plans.
     
  • Provides health care instruction and guidance to individuals, families, community groups and/or nursing home facilities in the management of their health care needs.
     
  • Maintains an effective working relationship with other members of the treatment team.
     
  • Prepares and submits records, reports, evaluations and forms. 
     
  • Participates in and make recommendations regarding quality assurance activities. 

Job Requirements

JOB REQUIREMENTS for Registered Nurse III
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. At least two years experience as a Registered Nurse in Community or Home Health Care Nursing OR a Bachelors degree or higher in Nursing and at least one year experience as a Registered Nurse in Community or Home Health Care Nursing OR a Masters degree or higher in Nursing.
  2. Possession of a Delaware Registered Nurse license OR multi-state compact license.
