Introduction

 

Treatment Accountability for Safer Communities (TASC) was created nationally in 1972 under the Drug Abuse and Treatment Act.

The Treatment Access Center (TASC) is the primary liaison between the Division of Substance Abuse and Mental Health and the criminal justice system. We assess, refer to treatment and provide case management services to offenders as they move through both the criminal justice and treatment systems. TASC services are provided statewide to offenders coming through any Delaware court. Assessments are conducted and treatment recommendations are provided to the court and other criminal justice officials for use in disposition. Once a case is engaged, TASC ensures that treatment placement occurs in a timely basis. TASC then follows the client as he/she progresses through treatment and into recovery.

Summary Statement

The incumbent is responsible for assessment, referral and case management of specialty court clients referred by Delaware courts.  Clients are in the community & Department of Correction.  Work includes: urine monitoring, front desk coverage, referring clients to the appropriate level of care, following client's progress, reporting to the court via status hearings, entering prisons for TASC client assessments, interviews, & reports, assisting TASC clients in accessing medical, housing, education, and other like services to support TASC, its clients or stakeholders & other duties as needed.

Essential Functions

  • Interviews client, family and/or professional sources to determine clients needs for a variety of social services which may include employment, job search and basic skills remediation.

  • Develops a comprehensive cost effective service plan utilizing in-house sources, referrals, contracted providers and/or institutions.

  • Conducts on-going monitoring and assessment of service delivery for optimum quality and efficiency, and authorizes and recommends changes and adjustments as needed.

  • Maintains comprehensive case records, summaries, statistical and narrative reports and completes required forms.

  • Establishes effective working relationship with clients, family, community and professional sources.

  • Provides specialized casework and counseling services in such areas as protective, adoption and options counseling.

  • Provides direct casework service through individual/group conferences, counseling and/or site visits.

  • Supplies information and responses to inquiries regarding services for clients, family, community and government agencies.

  • May provide supportive casework to clients prior to, during or after hospitalization based upon physicians diagnosis and recommended treatment.

    Job Requirements

    JOB REQUIREMENTS for Senior Social Worker/Case Manager
    Applicants must have education, training and/or experience demonstrating competence in each of the following areas:
    1. Six months experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individuals human service needs.
    2. Six months experience in making recommendations as part of a client's service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
    3. Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
    4. Six months experience in using an automated information system to enter, update, modify, delete, retrieve/inquire and report on data.
    5. Six months experience in narrative report writing.

    Additional Posting Information

    Due to a new requirement from the Centers for Medicare & Medicaid Services (CMS) the following mandate is effective January 27, 2022: As a condition of employment, employees providing health care services and/or support services in covered facilities must show proof that they have received the dose of a single J&J dose or the 1st dose of Moderna/Pfizer two-dose vaccine effective date January 27, 2022, and proof of the single J&J dose or the 2nd dose of Moderna/Pfizer two-dose vaccine by February. 28, 2022. Employees may not provide any care, treatment, or other services until proof is provided.  Based on CMS guidance, the date for compliance may extend into March due to the health care facilities high vaccination rate of staff.  If you accept a position with the facility, your Human Resources representative can provide more guidance.  The only exception to these requirements is for those employees granted medical or religious exemptions from the COVID-19 vaccine as recommended by the Centers for Disease Control and Prevention, which may require weekly COVID-19 testing.
