An incumbent in this position would work directly with the residents, their families, guardians or decision makers. They would be responsible for providing resident advocacy and promoting person centered care. They are an integral part of the Resident's Care Team, which operates to review person centered care plans with the residents, making sure that the plan continually meets the residents needs. They are responsible for providing counseling services as needed to the residents and their families as they adjust to their new environment and life changes. They must be able to complete interviews with the residents and their families to obtain necessary information for reports, assessments, case notes, outside services and guardianships. Finally, they are responsible for working with the discharging team in order to assist residents with being able to return to the community setting when able.

1. Possession of a Bachelors degree or higher in human services such as Social Work, Sociology, Special Education, Rehabilitation Counseling, Psychology or related field.
