Summary Statement

As a general overview, a class incumbent is responsible for case management in a long-term care setting.  The incumbent will be responsible for developing person-centered treatment plans and to perform as part of an interdisciplinary treatment team member.  This incumbent is an essential liaison for family members, providing referrals or resources when needed for the resident and coordinating with other professionals involved in the care of each resident.  Work will include periodic assessments, evaluation of progress toward treatment goals, coordination of the care plan meeting process, serving as an advocate for the resident and providing assistance wherever possible to promote quality of life so as to enhance the psychosocial, spiritual and emotional well-being of each resident.

Essential Functions

Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class.   • Provides direct casework services which includes emotional and social assessments, problem identification and develops a service plan to assist client and family in dealing with medical conditions, following medical recommendations and achieving maximum social and health adjustment.
  • Assesses financial eligibility of families seeking financial, medical or related services from the Division. Provides referral and follow up services to alternative sources for ineligible applicants. Assists client in obtaining same.
  • Provides individual and family counseling to clients; obtains and coordinates public, private or community medical, social and therapy services to meet client needs.
  • Reviews client's case history and evaluates client's potential to benefit from further medical and social treatments. Discusses case progress with multidisciplinary treatment team and prepares recommendations based on team findings.
  • Prepares social histories, client progress reports, evaluations and discharge summaries; Prepares and maintains a variety of records and files pertaining to caseload and files.
  • Provides medical social work consultation to division program staff, private medical professionals and/or hospital social service departments.

Job Requirements

JOB REQUIREMENTS for Senior Medical Social Work Consultant
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Three years experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  3. Three years experience in making recommendations as part of a clients service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  4. Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  5. Six months experience in narrative report writing.

Additional Posting Information

Due to a new requirement from the Centers for Medicare & Medicaid Services (CMS) the following mandate is effective January 27, 2022: As a condition of employment, employees providing health care services and/or support services in covered facilities must show proof that they have received the dose of a single J&J dose or the 1st dose of Moderna/Pfizer two-dose vaccine effective date January 27, 2022, and proof of the single J&J dose or the 2nd dose of Moderna/Pfizer two-dose vaccine by February. 28, 2022. Employees may not provide any care, treatment, or other services until proof is provided.  Based on CMS guidance, the date for compliance may extend into March due to the health care facilities high vaccination rate of staff.  If you accept a position with the facility, your Human Resources representative can provide more guidance.  The only exception to these requirements is for those employees granted medical or religious exemptions from the COVID-19 vaccine as recommended by the Centers for Disease Control and Prevention, which may require weekly COVID-19 testing.
