The incumbent in this position will further advance the Division of Public Health’s efforts related to improving population health, specifically working with communities on policy, system and environmental strategies that improve the social determinates of health. The main purpose of this job is to work with communities to understand, develop and implement policy systems and environmental (PSE) change strategies within their communities. This position will provide education and technical assistance in the area of PSE to strengthen communities’ abilities to reduce chronic diseases and improve health in their areas. The incumbent in this position will become the County Level expert on communities within a specific geographic area and the Public Health and Social Determinant of Health issues affecting these communities. The incumbent will guide agency and coalition partners through health planning process to improve local outcomes, develop recommendations for utilization of existing community resources and adopt new action plans to improve health.
