Summary Statement

The Transition Specialist is an educational advocate serving as a liaison between students who are transitioning out of placement in a DSCYF educational facility and the school or program into which the student will be enrolling or entering.  They will also be responsible for assisting with credit recovery and the administration of career interest inventories to supplement student portfolios.

Essential Functions

Essential Functions To perform this job successfully, the incumbent must be able to perform the essential duties and responsibilities in a satisfactory manner.   • Serves as a liaison between teaching staff and staff at the facility.
  • Written communication and documentation is central to this position.
  • Develops and implements a comprehensive academic transition plan, including a graduation plan, in conjunction with students, families, service providers and the receiving school for students returning to local districts.
  • Provides coordination of curricula between DSCYF and the local receiving school to ensure continuity of instruction and provides an educational summary that includes a clear description of student's academic skills to receiving schools and programs.
  • Collaborates/communicates with members of the Education Unit, school districts, other service providers, and parents.
  • Complies with Federal, State and Local Regulations, Policies and Procedures.
  • Provides precise information to receiving schools about students special needs (IEP or 504 plans).

Job Requirements for Teacher Special Education

Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Must hold (or be eligible for) State of Delaware Certification in Special Education and/or School Counseling.
  2. Bachelor’s and/or Master’s Degree in Education from an accredited University.
  3. Demonstrated proficiency in technology application in a classroom setting.
  4. Demonstrated evidence on knowledge regarding best practices in effectively promoting student engagement while increasing student achievement.
  5. Experience working with alternative populations/at-risk youth.
  6. Knowledge of Learning Focused instruction.

License,Registrations and Certifications

Applicant will submit the following at time of interview:

  1. Copy of Professional Status Certification or License, and/or Alternative Routes to Certification (ARTC) Letter in Special Education and/or School Counseling.
  2. Documentation of Praxis-I scores.

Conditions of Hire

Criminal background check:  A satisfactory criminal background check is required as a condition of hire.  The recruiting agency may require the applicant to pay for the criminal background check as part of the conditional offer of hire.    

Pre-employment Drug Testing:  Upon a conditional offer, you must undergo pre-employment drug testing as part of the hiring process.

This position is a classification organized under an exclusive bargaining representative (labor organization).  The candidate selected for this position shall be asked to join and pay dues to the labor organization or may, decline joining the labor organization and not pay dues or a fee.  The labor organization has been elected by employees as their representative for collective bargaining and other work related purposes.
