Description of Occupational Work

This class series uses two levels in the Health and Human Services occupational group, Human Services occupational series and describes medical social work in providing case management services to clients of one of the following Public Health or Social Services service areas: Home Health, Adult Health and Child Health programs. Specialization is typically in areas of medical social work such as prenatal, pediatric high-risk and family services, handicapped services (orthopedics, neurology, otology and audiology, and cleft palate).

Essential Functions

Essential functions are fundamental, core functions common to all positions in the class series and are not intended to be an exhaustive list of all job duties for any one position in the class. Since class specifications are descriptive and not restrictive, incumbents can complete job duties of similar kind not specifically listed here.

  • Provides direct casework services which includes emotional and social assessments, problem identification and develops a service plan to assist client and family in dealing with medical conditions, following medical recommendations and achieving maximum social and health adjustment.
     
  • Assesses financial eligibility of families seeking financial, medical or related services from the Division. Provides referral and follow up services to alternative sources for ineligible applicants. Assists client in obtaining same.
     
  • Provides individual and family counseling to clients; obtains and coordinates public, private or community medical, social and therapy services to meet client needs.
     
  • Reviews client's case history and evaluates client's potential to benefit from further medical and social treatments. Discusses case progress with multidisciplinary treatment team and prepares recommendations based on team findings.
     
  • Prepares social histories, client progress reports, evaluations and discharge summaries; Prepares and maintains a variety of records and files pertaining to caseload and files.
     
  • Provides medical social work consultation to division program staff, private medical professionals and/or hospital social service departments.

Levels of Work

Medical Social Work Consultant

This is the full performance level of medical social work case management services, participating as a member of a medical treatment team.

  • Performs full range of essential functions.


Senior Medical Social Work Consultant

This is the advanced level responsible for directly providing specialized case management services and for social services consultation services to professional medical care staff in the public and private sector.

  • This level is differentiated from the lower level of this series by the greater independent judgement and the emphasis on providing consultative services.
  • Participates as a treatment team member to provide a social assessment and plan as part of the overall patient/client medical plan. Provides follow-up with patient and community resources to ensure coordination and consistency of medical/social treatment.

Knowledge, Skills and Abilities

The intent of the listed knowledge, skills and abilities is to give a general indication of the core requirements for all positions in the class series; therefore, the KSA's listed are not exhaustive or necessarily inclusive of the requirements of every position in the class.

  • Knowledge of the principles, practices and techniques of medical social work.
  • Knowledge of the social factors affecting the disease process or health concerns.
  • Knowledge of case management practices and techniques.
  • Knowledge of individual and family counseling methods and techniques.
  • Knowledge of community agencies and resources for educational, health and social services.
  • Skill in interviewing to obtain client case histories
  • Skill in counseling individuals and families.
  • Ability to coordinate case management for multidisciplinary treatment teams in regards to social aspects of the clients medical or health problem.
  • Ability to provide training and guidance in the resolution of medical social work case management problems.


In addition to the above knowledge, skills and abilities, the Senior Medical Social Work Consultant requires

  • Knowledge of social work methods and techniques used to eliminate adverse factors impacting on the health care, management process of clients.
  • Skill in providing consultative services.

 

Job Requirements

JOB REQUIREMENTS for Senior Medical Social Work Consultant
Applicants must have education, training and/or experience demonstrating competence in each of the following areas:

  1. Possession of a Bachelors degree or higher in Behavioral or Social Science or related field.
  2. Three years experience in case management which includes assessing, planning, developing, implementing, monitoring, and evaluating options and services to meet an individual's human service needs.
  3. Three years experience in making recommendations as part of a clients service plan such as clinical treatment, counseling, or determining eligibility for health or human services/benefits.
  4. Six months experience in interpreting laws, rules, regulations, standards, policies, and procedures.
  5. Six months experience in narrative report writing.
