The State of Connecticut, Department of Developmental Services (DDS) - North Region's, Private Services Division - has one (1) current Full-Time job opportunity for a Clinical Nurse Coordinator based out of the East Hartford Office. This is a full-time, 35 hour per week position, with a schedule of:
  • Monday - Friday: 8:30 am - 4:00 pm
  • Regular Days Off: Saturday & Sunday
  • However, work schedule flexibility may be required in order to meet the demands of the job
In this role, the selected candidate will be expected to provide case support through nursing clinical consultation to agency staff. We would empower you to assist staff in the decision-making process as it relates to medical and health issues of individuals in DDS’s care. Additionally, among other duties assigned, we would have you:
  • Provide nursing consultation
  • Participate in home visitations with other staff
  • Attend physician appointments and hospital visits as needed
  • Work collaboratively with community-based healthcare providers in facilitating care and addressing the health needs of individuals in our care

PURPOSE OF JOB CLASS (NATURE OF WORK)

In a state facility this class is accountable for coordinating the various facets of clinical programs involving multiple units OR for serving as liaison between the facility and community programs and placement settings.

EXAMPLES OF DUTIES

The Clinical Nurse Coordinator’s responsibilities include but is not limited to the following:
  • Conducting site visits for enhanced monitoring of nursing/medical concerns and nursing delegation
  • Review of regional quarterly medication audits and communicate with the Regional Health Services Director and Nurse Consultant regarding areas of concern
  • Assist private provider new nurses with implementing nursing delegation
  • Plan, review and evaluate the coordination, assessment and monitoring of health services pertaining to individuals residing in private agency community residences
  • Assist the Nurse Consultant with insuring DDS compliance with OBRA (Omnibus Budget Reconciliation Act, also known as the Nursing Home Reform Act of 1987) regulations and CMS (Centers for Medicare and Medicaid Services) oversight
  • Assess safety and quality of care being provided and assist in identifying alternative placements to decrease the number of DDS consumers receiving LTC in skilled nursing facilities
  • Provide a nursing summary of all site visits/monitoring and any nursing instruction or assistance provided to private agency nurses
  • Maintain accurate written and/or electronic data and reports
  • May provide nursing input to Planning and Support Teams (PST)
  • Assist with the review of investigations related to nursing/medical issues; participate in the DDS Regional Nurse On-Call System for Public Programs
  • At times, duties may include providing temporary assistance with nursing/medical needs and assessments for individuals assigned to DDS Public Programs , Individual Family Support (IFS) or Community Companion Homes (CCH)
  • The Clinical Nurse Coordinator (CNC) is a member of the Regional Mortality Review Committee, responsible for reviewing nursing/medical/residential records of deceased persons to address concerns and improve care and outcomes for all individuals served by DDS
  • The CNC attends and may contribute to the regional Private Sector Nurses’ quarterly meetings and Public Program Nurses’ monthly meetings

KNOWLEDGE, SKILL AND ABILITY

Knowledge of nursing principles and clinical therapeutic models of individual care with emphasis on interdisciplinary team approaches; knowledge of individual treatment planning including impact of medical insurance program requirements for reimbursement; knowledge of structure of services for individuals and their families in institutional and community oriented settings; knowledge of aftercare facilities; considerable interpersonal skills; oral and written communication skills.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Three (3) years of professional nursing experience.

MINIMUM QUALIFICATIONS - SPECIAL EXPERIENCE

One (1) year of the General Experience must have been at the level of Head Nurse working with the type of individuals serviced by the facility.

DEPARTMENT OF DEVELOPMENTAL SERVICES: One (1) year of the General Experience must have been providing consultation and evaluating the assessment of nursing services provided to individuals with intellectual disabilities.

MINIMUM QUALIFICATIONS - SUBSTITUTIONS ALLOWED

  1. A Bachelor's degree in nursing may be substituted for one (1) year of the General Experience.
  2. A Master's degree in nursing may be substituted for one (1) additional year of the General Experience.

PREFERRED QUALIFICATIONS

Preference will be given to applicants who possess the following:
  • Experience working with the medical and behavioral needs of individuals with Intellectual and/or Developmental Disabilities across the life span
  • Experience working with individuals with intellectual disabilities and/or autism
  • Experience with clinical documentation including documenting events in writing
  • Experience working in community and family home settings and coordinating in home supports   
  • Experience reviewing health information and formulating reports based on that information
  • Experience utilizing Microsoft Office products including, Outlook, Word, Excel and TEAMS  
  • Experience communicating in writing and verbally with individuals of varying populations and skill levels

SPECIAL REQUIREMENTS

  1. Incumbents in this class must possess and retain a license as a registered nurse in Connecticut.
  2. Incumbents in this class may be required to travel.
