The State of Connecticut, Department of Social Services (DSS) is recruiting for a Supervising Community Nurse Coordinator in the Community Options Unit for the Division of Health Services, located at 55 Farmington Avenue, Hartford, CT 06105. This full-time (40 hour per week) position has a Monday through Friday schedule. The first shift hours are 8:00 am to 4:30 pm.

In order to be considered for this job opening, you must be a current State of CT employee of the Dept. of Social Services, who has permanent state status*, and meet the Minimum Qualifications as listed on the job opening. You must specify your qualifications on your application.

*For employees in the classified service, permanent status is obtained after successful completion of a working test period. For employees in the unclassified service, permanent status is obtained after serving in a position for at least six months (full-time or full-time equivalent).

PURPOSE OF JOB CLASS (NATURE OF WORK)

In the Department of Social Services, Home and Community Based Services unit this class is accountable for supervising a professional nursing staff engaged in providing consultation to access agencies who provide case management to consumers and their families regarding social services program-based healthcare services.

EXAMPLES OF DUTIES

Schedules, assigns, oversees and reviews work of staff; determines priorities and plans unit work; establishes and maintains unit procedures; develops or makes recommendations on development of policies and standards; motives and evaluates staff; provides staff training and assistance; conducts performance evaluations; assesses, evaluates and monitors quality of documentation, records and information concerning community health services; analyzes and evaluates home and community base services provided; prepares reports and correspondence; acts as a liaison with other units, agencies and outside organizations regarding unit policies and procedures; may conduct on site visits to facilities for quality of care and or compliance with federal and state regulations; may testify in court on state and federal policy and regulation; performs related duties as required.

KNOWLEDGE, SKILL AND ABILITY

Considerable knowledge of relevant state and federal laws, statutes and regulations; considerable knowledge of relevant agency policies and procedures; considerable knowledge of principles and practices of nursing care administration for a variety of specialties with varied settings; considerable knowledge of managed care practices; considerable knowledge of principles and practices pertinent to community processes; considerable knowledge of state, federal and local health agencies and programs; considerable interpersonal skills; considerable oral and written communication skills; considerable ability to problem solve; considerable ability to understand and communicate health programs; considerable ability to plan, implement and evaluate programs; ability to utilize computer software; supervisory ability.

MINIMUM QUALIFICATIONS - GENERAL EXPERIENCE

Five (5) years of experience as a registered professional nurse in a position involving home healthcare, nursing homes or other community based nursing program.

MINIMUM QUALIFICATIONS - SPECIAL EXPERIENCE

Two (2) years of the General Experience must have been at the level of Community Nurse Coordinator.

PREFERRED QUALIFICATIONS

Preference will be given to candidates with the following experience:
  • Supervising professional nursing staff
  • Providing clinical consultation
  • Supervising a diverse team
  • Collaborating with providers, agencies and organizations
  • Working with Medicaid waiver services
  • Working with web-based computer systems
  • Training in community based long term services and supports
  • With utilization review and prior authorizations

SPECIAL REQUIREMENTS

1. Incumbents in this class must possess and retain a license as a registered professional nurse in Connecticut.
2. Incumbents in this class may be required to travel.
3. Incumbents in this class may be required to possess a valid Motor Vehicle Operator's license.
